研究目的：

建立中西医结合早期防治AD的疗效和安全性评价体系；制定AD痴呆前阶段中西医结合的诊疗方案和路径；培养一批中西医结合防治AD专家队伍。  

Objectives of Study：

To establish the efficacy and safety evaluation system for early prevention and treatment of Alzheimers disease (AD) with integrated traditional Chinese and western medicine;To develop the diagnosis and treatment scheme and path for prodromal stage of AD using integrated traditional Chinese and western medicine;To cultivate a team of experts in the prevention and treatment of AD with integrated traditional Chinese and western medicine.

纳入标准：

Inclusion criteria

排除标准：

1. 有明确的脑血管卒中病史，且发病时有明确的神经功能缺损的症状或体征，神经影像遗留有相对应的责任病灶；
2. 严重的脑白质病变（Fazekas 评分≥3 分）；
3. 患有任何原因导致的意识障碍；
4. 或肢体残疾不能完成神经心理检查者；
5. 现患有相关抑郁症（汉密顿抑郁量表评分＞8 分）或精神分裂症等精神疾病者；
6. 患有酒精中毒史，或药物成瘾史，或脑外伤、癫痫、脑炎、正常颅压脑积水等可引起认知障碍的神经系统疾病者；
7. 患有可能导致轻度认知障碍的系统性疾病（如肝肾功能不全、内分泌疾病、维生素缺乏）；
8. 存在异常实验室指标：肝功能(ALT、AST)超过正常范围上限2倍，肾功能(Cr)超过正常范围上限 2倍，肌酸激酶超过正常范围上限2 倍，白细胞计数＜4.0×10^9或血小板＜100×10^9，血红蛋白＜100g/L，随机血糖值超过正常范围上限的1.2倍；
9. 妊娠试验阳性或在哺乳期的女性受试者及无法采取有效的避孕措施或有生育计划的受试者；
10. 正在参加其它药物临床研究者；
11. 本研究开始前1个月内使用过影响认知功能的药物，如：胆碱酯酶抑制剂、美金刚、拟胆碱药物、抗精神病药物、抗焦虑药、催眠药、促智药（包括中国传统的草药和丸药，如苁蓉益智胶囊、健脑安等）；
12. 筛选前6个月内接受过甘露特钠胶囊、养血清脑丸治疗的患者；
13. 对养血清脑丸、甘露特钠胶囊过敏或有不良反应史者；
14. 无法进行头颅MRI影像学检查者；
15. 研究者认为不适合参与试验者。

Exclusion criteria：

1. There is a clear history of cerebrovascular stroke, and there are clear symptoms or signs of neurological deficit at the time of onset, and there are corresponding responsible lesions left on neuroimaging;
2. Severe white matter lesions (Fazekas score >= 3);
3. Suffering from any disorder of consciousness;
4. Those with physical disabilities who cannot complete the neuropsychological examination;
5. Those who currently suffer from related depression (score of Hamilton Depression Scale>8) or schizophrenia and other mental diseases;
6. Those with a history of alcoholism, drug addiction, or neurological diseases that can cause cognitive impairment such as traumatic brain injury, epilepsy, encephalitis, normal intracranial pressure hydrocephalus;
7. Have systemic diseases that may lead to mild cognitive impairment (such as liver and kidney insufficiency, endocrine diseases, vitamin deficiencies);
8. Abnormal laboratory indicators: liver function (ALT, AST) exceeds 2 times the upper limit of the normal range, renal function (Cr) exceeds 2 times the upper limit of the normal range, creatine kinase exceeds 2 times the upper limit of the normal range, and white blood cell count < 4.0 x 10 ^9 or platelets <100x10^9, hemoglobin <100g/L, random blood glucose value exceeding 1.2 times the upper limit of the normal range;
9. Female subjects with positive pregnancy test or breastfeeding and subjects who are unable to take effective contraceptive measures or have childbearing plans;
10. Participating in other drug clinical investigators;
11. Use of drugs that affect cognitive function within 1 month before the start of this study, such as: cholinesterase inhibitors, memantine, cholinergic drugs, antipsychotics, anxiolytics, hypnotics, nootropics (including traditional Chinese herbs and pills, such as Cistanche Yizhi Capsule, Jiannaoan, etc.);
12. Patients who have received Mannat Sodium Capsules and Yangxuenao Pills within 6 months before screening;
13. Those who are allergic to Yangxue Nao Pill and Mannat Sodium Capsule or have a history of adverse reactions;
14. Those who are unable to perform head MRI imaging examination;
15. The investigators believe that they are not suitable to participate in the trial.

利用SAS9.4或以上版本统计软件采用分层区组随机化方法产生随机数。共有19个中心参与，每个中心作为分层因素，区组长度为4，将各个受试病例按照1：1：1的比例随机分入中西医结合治疗组、甘露特钠胶囊治疗组和对照组中。随机数具有重现性。所设定的区组长度及种子数等参数记录在随机编码表中。 借助SAS统计分析系统，产生480受试者所接受干预的随机安排，即列出流水号为001～480所对应的治疗分配（随机编码表）。通过交互式网页应答系统(interactive web response system, IWRS)实施分层区组随机化使受试者被随机分派到相应的试验组或对照组。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Random numbers are generated using stratified block randomization method with SAS 9.4 or above statistical software. A total of 19 centers participated, with each site serving as a stratification factor and a block size of 4. Subjects will be randomized in a 1:1:1 ratio.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

所有人口统计学、临床、神经心理学及影像实验室检查数据均先由各参研单位研究人员登记在病例报告表（CRF），相关人员进行初步审查及核对后录入数据网络管理平台，通过多中心网络管理平台，进行多中心录入数据的管理和共享。主研究单位具有可访问全部研究单位数据权限，各参研单位仅具有各自单位数据访问权限。成都赛美斯系统将作为EDC系统在项目中被使用。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

All data including demographic, clinical, neuropsychological, imaging and laboratory data are first registered in the case report form (CRF) by the researchers of each participating unit, and the relevant personnel will enter the data network management platform after preliminary review and verification. The multi-center input data will be shared via the multi-center network management platform. The main research unit has the right to access the data of all research units, and each participating unit only has the data access right of its own unit. CIMS (Chengdu, China) will be used as electronic data capture (EDC) system in the project.
