主要目的：评价斑点追踪技术定量膈肌的纵向应变的可行性和可重复性，以及对呼吸负荷卸载的反应性。  

Objectives of Study：

Main objective: To evaluate the feasibility and reproducibility of speckle tracking techniques to quantify longitudinal strain of the diaphragm and responsiveness to respiratory load unloading.

药物成份或治疗方案详述：

第一部分纳入25例健康志愿者，第二部分纳入20名患者。 第一部分：志愿者平卧位（30°）休息10min后由操作者1进行平静呼吸状态下测量膈肌纵向应变和增厚分数，在最大深吸气下再次测量膈肌纵向应变和增厚分数。志愿者休息半小时后由操作者2进行平静呼吸状态下测量膈肌纵向应变和增厚分数，在最大深吸气下再次测量膈肌纵向应变和增厚分数。 第二部分：机械通气患者满足更改为PSV模式的条件后更改为PSV模式，调整PEEP：5cmH20，PS:5cmH20、10cmH20、15cmH20、20 cmH20不同压力支持下膈肌纵向应变和增厚分数。 

Description for medicine or protocol of treatment in detail：

The first part included 25 healthy volunteers, and the second part included 20 patients. Part 1: Volunteers rest in a supine position (30°) for 10 minutes, and operator 1 measures the longitudinal strain and thickening of the diaphragm under a calm breathing state, and then measures the longitudinal strain and thickening of the diaphragm again under maximum deep inhalation. Thick scores. After the volunteers rested for half an hour, the operator 2 measured the longitudinal strain and thickening score of the intercostal muscles under a calm breathing state, and again measured the longitudinal strain and thickening scores of the diaphragm under the maximum deep inhalation. Part II: the mechanical ventilation patients change to PSV mode after meeting the conditions for changing to PSV mode, adjust PEEP: 5cmH20, PS: 5cmH20, 10cmH20, 15cmH20, 20 cmH20 different pressure supports longitudinal strain and thickening score of diaphragm. 

纳入标准：

Inclusion criteria

排除标准：

第一部分：
1.年龄<18岁；
2.有吸烟史。
第二部分：
1.年龄<18岁；
2.既往存在神经肌肉疾病；
3.孕妇；
4.存在肋骨骨折；
5.无自主呼吸；
6.正在使用神经肌肉阻滞剂；
7.检查部位存在胸腔闭式引流管；
8.单侧或双侧膈肌麻痹。

Exclusion criteria：

First part:
1. Age <18 years old;
2. Have a history of smoking.
Second part:
1. Age <18 years old;
2. Preexisting neuromuscular disease;
3. Pregnant women;
4. There is a rib fracture;
5. No spontaneous breathing;
6. Are using neuromuscular blocking agents;
7. There is a closed chest drainage tube at the inspection site;
8. Unilateral or bilateral diaphragmatic paralysis.
