研究目的：

本研究的主要目标在于通过规范的前瞻性临床试验，客观、准确地评估周围神经三联减压术式可否有效缓解下肢痛性DPN患者的神经病理性疼痛症状，同时评估周围神经三联减压术式可否缓解下肢痛性DPN患者并存的感觉减退症状、神经电生理指标（神经传导速度及振幅等）以及抑郁焦虑等心理障碍状态，以期客观、较全面地评价周围神经三联减压术式在治疗下肢痛性DPN患者中的作用。

Objectives of Study：

The research aims to objectively and comprehensively evaluate the efficacy of triple peripheral nerve decompression for neuropathic pain of diabetic peripheral neuropathy in lower extremities. The dysesthesia symptoms, nerve electrophysiological indexes (nerve conduction velocity and amplitude) and the psychological status were also assessed.

纳入标准：

Inclusion criteria

1. According to the 1999 WHO diagnostic criteria for type 2 diabetes, aged 40 to 70 years old;
2. Michigan Diabetes Peripheral Nerve Screening (MNSI) score > 2;
3. The hypoglycemic regimen is stable, maintaining fasting blood glucose <8mmol/L, 2h postprandial blood glucose <15mmol/L, and glycosylated hemoglobin <8.5%;
4. Symmetric pain in both lower extremities, distributed in the sensory areas of the common peroneal nerve, posterior tibial nerve and deep peroneal nerve, pain symptoms persisting for more than 6 months, bilateral pain visual analogue scale (VAS) > 4 points, difference < 2 points , the average VAS score in one week was >4 points;
5. There are positive Tinel signs on both sides, including but not limited to the following sites: the lateral peroneal canal of the knee (common peroneal nerve), the medial tarsal canal (posterior tibial nerve) and between the first and second metatarsal bones anterior (deep peroneal nerve);
6. Nerve electrophysiology indicates that there is more than one nerve (common peroneal nerve, posterior tibial nerve and deep peroneal nerve) in both lower extremities with slow conduction velocity (>=2 standard deviations), which may be accompanied by decreased amplitude.

排除标准：

1.合并存在其他可能导致周围神经病变的伤病或药物服用史，如恶性贫血、甲状腺功能减退、下肢血管疾病（足背动脉波动消失或减弱，静脉曲张等）；下肢外伤史或手术史；药物使用史如长春新碱、苯妥英钠；重金属中毒史等；
2.入组1年内存在酒精或药物成瘾、滥用史；
3.腰椎狭窄或盘源性神经病变；
4.各类原因引起的下肢水肿；
5.严重的出血性疾病或出血倾向的患者；
6.心、肺、肝、肾等器官功能障碍难以耐受手术或药物者；
7.精神障碍等因素影响依从性的患者；
8.风湿和类风湿疾病等；
9.正参加其它临床试验的患者。

Exclusion criteria：

1. Combined with other injuries or history of drug use that may lead to peripheral neuropathy, such as pernicious anemia, hypothyroidism, lower extremity vascular disease (disappearance or weakening of dorsal foot artery fluctuations, varicose veins, etc.); history of lower extremity trauma or surgery; History of drug use such as vincristine, phenytoin; history of heavy metal poisoning, etc.;
2. There is a history of alcohol or drug addiction and abuse within 1 year of enrollment;
3. Lumbar stenosis or discogenic neuropathy;
4. Lower extremity edema caused by various reasons;
5. Patients with severe bleeding disorders or bleeding tendency;
6. Those with heart, lung, liver, kidney and other organ dysfunction that cannot tolerate surgery or drugs;
7. Patients with mental disorders and other factors that affect compliance;
8. Rheumatism and rheumatoid diseases, etc.;
9. Patients who are participating in other clinical trials.
