研究目的：

主要目的：本项目旨在建立规范化、高质量的呼吸衰竭及其相关急危重症队列和样本库，通过对慢性阻塞性肺疾病急性加重患者并发II型呼吸衰竭时使用无创机械通气的预后模型构建及对无创机械通气预后模型的前瞻性队列验证，建立无创机械通气，经鼻高流量湿化氧疗个体化治疗方案，提高呼吸衰竭综合防控水平。

Objectives of Study：

Main purpose: This project aims to establish a standardized, high-quality respiratory failure and its related emergency and critical illness cohort and sample library, through the construction of a prognostic model of non-invasive mechanical ventilation in patients with acute exacerbation of chronic obstructive pulmonary disease and type II respiratory failure and the use of non-invasive The prospective cohort verification of the prognosis model of mechanical ventilation, the establishment of non-invasive mechanical ventilation, and the individualized treatment plan of nasal high-flow humidified oxygen therapy to improve the comprehensive prevention and control level of respiratory failure.

纳入标准：

Inclusion criteria

1. All patients meet the diagnostic criteria for chronic obstructive pulmonary disease in GOLD 2021:
(1) Symptoms: respiratory symptoms such as long-term chronic cough, expectoration, shortness of breath or dyspnea, and systemic symptoms such as weight loss, loss of appetite, cyanosis, mental depression or anxiety;
(2) Risk factors: long-term heavy smoking, exposure to harmful gases and particles such as occupational dust, smog, indoor biofuels, low socioeconomic status, human immunodeficiency virus (HIV) infection, etc.
2. Severe dyspnea and poor response to initial treatment;
3. Changes in the state of consciousness (such as confusion, lethargy, coma, etc.);
4. The results of blood gas analysis meet the diagnostic criteria for type II respiratory failure: that is, at sea level atmospheric pressure, breathing room air under resting conditions, and excluding intracardiac anatomical shunt and primary cardiac output reduction, arterial blood The partial pressure of oxygen (PaO2) is lower than 8kPa (60mmHg), and the partial pressure of carbon dioxide in the arterial blood is higher than 6.65kPa (50mmHg).

排除标准：

1.有需要建立人工气道而拒绝接受、最终导致死亡的病例
2.入院时，存在严重的高碳酸血症（PaCO2＞75mm Hg）；
3.入院后24 小时内行气管插管或气管切开的患者；
4.意识障碍或合并精神类疾病的患者；
5.严重心功能不全、肝肾功能不全的患者；
6.合并哮喘、肺结核、间质性肺疾病、胸廓畸形、恶性肿瘤、重症肌无力的患者；
7.近期手术、外伤、重症感染的患者；
8.辅助通气治疗中断；
9.不配合抽血监测实验指标。

Exclusion criteria：

1. Those who need to establish an artificial airway but refuse to accept it and eventually lead to death
2. Severe hypercapnia (PaCO2>75mm Hg) at the time of admission;
3. Patients who underwent tracheal intubation or tracheotomy within 24 hours after admission;
4. Patients with disturbance of consciousness or combined mental illness;
5. Patients with severe cardiac insufficiency, liver and kidney insufficiency;
6. Patients with asthma, pulmonary tuberculosis, interstitial lung disease, thoracic deformity, malignant tumor, myasthenia gravis;
7. Patients with recent surgery, trauma, or severe infection;
8. Interruption of assisted ventilation therapy;
9. Do not cooperate with blood sampling to monitor experimental indicators.
