研究目的：

探讨乌司他丁能否通过抗炎作用安全有效地降低老年患者下肢关节置换术后谵妄的发生率、住院时间,观察血清炎性因子（S100β, TNF-α,IL-6,IL-1β）的变化情况，为乌司他丁的临床应用及术后谵妄的预防提供一种新思路。  

Objectives of Study：

Explore whether ulinastatin can safely and effectively reduce the incidence of delirium and the duration in hospital in elderly patients with lower limb joint replacement by playing a role in its anti-inflammatory effects, observing the serum inflammatory cytokines' (S100 beta, TNF-alpha, IL-6, IL-1 beta) changes, providing a new way for the clinical application of ulinastatin and the prevention of postoperative delirium.

纳入标准：

Inclusion criteria

排除标准：

①存在精神疾病或有精神病家族史 ②服用抗抑郁药或安定类药物 ③听视觉障碍④文盲 ⑤严重电解质紊乱 ⑥严重酗酒 ⑦既往有心血管和（或）神经外科手术史 ⑧患有脑血管疾病伴有后遗症 ⑨有明显心、肺、肝、肾功能不全 ⑩MMSE评分低于相应文化程度的最低评分（ 小学≦２０分， 中学及以上≦２３分）

Exclusion criteria：

1. mental disease or have a family history of mental illness;
2. taking antidepressant or antipsychotic drugs;
3. Auditory or/and visual disturbances;
4. illiteracy;
5. serious electrolyte disorder;
6. heavy drinking;
7. a history of cardiovascular and (or) nerve surgery;
8. Patients with cerebrovascular disease and accompanied by sequelae;
9. have obvious heart, lung, liver and renal insufficiency;
10. MMSE score lower than the corresponding cultural level of the lowest score (<=20 points of primary school, middle school and above <=23 points).
