研究目的：

探究米拉贝隆联合盐酸坦索罗辛对于BPH相关LUTS/OAB的疗效及安全性。  

Objectives of Study：

Explore the efficacy and safety of mirabegron combined with tamsulosin hydrochloride for BPH-related LUTS/OAB.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

Inclusion criteria

排除标准：

1.其他原因导致膀胱出口梗阻，尿道狭窄；
2.神经源性原因引起的逼尿肌过度活动；
3.存在尿路感染证据（尿常规示：WBC（+）），控制感染可以参与实验；既往反复尿路感染；
4.泌尿系结石；
5.慢性炎症，如间质性膀胱炎、前列腺炎；
6.接受OAB非药物治疗，2周内应用毒蕈碱受体拮抗剂；
7.过去12个月有膀胱灌注治疗，如肉毒杆菌；
8.药物：5α还原酶抑制剂应用；血管加压素、去氨加压素应用；选择性5羟色胺再摄取抑制剂、精神类药物应用；
9.临床症状明显的心血管或脑血管疾病；
10.平均QTcF间期>450msec（男性）或者有QT间期延长的风险；
11.临床明显的异常12导联心电图；
12.严重高血压（>180/110mmHg）；
13.中重度肝损伤（Child-Pugh分级B或C； AST和/或ALT>2倍上限值,或GGT>3倍上限值,或总胆红素>2倍上限值。）；
14.严重肾损伤（肾小球滤过率<30ml/min/1.73m^2，血肌酐>150μmol/L；
15.留置导尿或者练习间歇性自行导尿患者；
16.无法控制的闭角性青光眼，胃潴留，严重溃疡性结肠炎或克罗恩病，中毒性巨结肠，重症肌无力或抗胆碱治疗的禁忌证；
17.对盐酸坦索罗辛、米拉贝隆过敏；
18.泌尿系统恶性肿瘤；
19.前列腺根治术后，经尿道前列腺电切术后，尿道扩张术后；
20.PSA:4-10 ng/ml时，需要排除前列腺癌。（(1)直肠指诊阴性；(2)经直肠前列腺超声阴性；(3)前列腺穿刺活检阴性）；
21.PVR>300ml；
22.压力性尿失禁，或混合性尿失禁，而压力性尿失禁占主导；
23.饮酒或者药物滥用史；
24.4周内参加过其他临床试验。

Exclusion criteria：

1.Other reasons cause bladder outlet obstruction and urethral stricture;
2.Excessive activity of the detrusor muscle caused by neurogenic causes;
3.There is evidence of urinary tract infection (Urine routine shows: WBC(+)); infection control can participate in experiments; Previous recurrent urinary tract infections;
4.Urinary calculi;
5.Chronic inflammation, such as interstitial cystitis and prostatitis;
6.Receive OAB non-drug treatment, and apply muscarinic receptor antagonist within 2 weeks;
7.Have had bladder perfusion therapy in the past 12 months, such as botulinum;
8.Drugs: application of 5α reductase inhibitors; application of vasopressin and desmopressin; application of selective serotonin reuptake inhibitors and psychotropic drugs;
9.Cardiovascular or cerebrovascular diseases with obvious clinical symptoms;
10.The average QTcF interval> 450msec (male) or there is a risk of QT interval prolongation;
11.Clinically obvious abnormal of 12-lead electrocardiogram;
12.Severe hypertension (>180/110mmHg);
13.Moderate to severe liver injury (Child-Pugh grade B or C; AST and/or ALT>2 times the upper limit, or GGT>3 times the upper limit, or total bilirubin>2 times the upper limit.);
14.Severe kidney injury (Glomerular filtration rate <30ml/min/1.73m^2, blood creatinine>150μmol/L
15.Patients with indwelling catheterization or practicing intermittent self-catheterization;
16.Uncontrollable angle-closure glaucoma, gastric retention, severe ulcerative colitis or Crohn's disease, toxic megacolon, myasthenia gravis or contraindications for anticholinergic therapy;
17.Allergic to Tamsulosin Hydrochloride or Mirabegron;
18.Malignant tumors of the urinary system;
19.After radical prostatectomy, after transurethral resection of the prostate, after urethral dilation;
20.PSA: 4-10 ng/ml, prostate cancer needs to be excluded. ((1) Negative digital rectal examination; (2) Negative transrectal ultrasound of the prostate; (3) Negative prostate biopsy);
21.PVR >300ml;
22.Obvious stress urinary incontinence, or mixed urinary incontinence, and stress urinary incontinence dominates;
23.History of drinking or drug abuse;
24.Participated in other clinical trials within 4 weeks;
