研究目的：

针对老年慢病和老年综合征的特殊营养需求和生理状态进行营养与运动干预的交互作用研究，建立适合不同生理阶段老年人群体及个体的运动、营养干预关键技术。就模式推广的可行性、有效性、可持续性及社会经济效益等方面进行科学合理的综合评价，构建有特色的主动健康应用示范社区。建立适合国人老年健康促进方案，形成具有区域特色的老年健康促进中国方案。

Objectives of Study：

To study the interaction between nutrition and exercise intervention in chronic disease and geriatric syndrome. Establish the key techniques of exercise and nutrition intervention for elderly groups and individuals at different physiological stages. Make a scientific and reasonable comprehensive evaluation on the feasibility, effectiveness, sustainability and social economic benefits. Build a distinctive active health application demonstration community. Establish an elderly health promotion program suitable for Chinese people, and form a Chinese program for elderly health promotion with regional characteristics.

Inclusion criteria

排除标准：

1.重要脏器（心、肺、肾等）功能严重障碍者；
2.其他原因不能安全参与评估者；
3.声明不能完成随访过程者。

Exclusion criteria：

1. Those with severe dysfunction of important organs (heart, lung, kidney, etc.);
2. Those who cannot safely participate in the assessment for other reasons;
3. Those who declare that they cannot complete the follow-up process.
