研究目的：

1.探讨麻醉恢复室内呼吸锻炼对术后肺部并发症的影响，为拔管后早期呼吸锻炼的意义提供高级别的循证医学证据。 2.探讨拔除气管导管后早期在麻醉恢复室内进行呼吸锻炼对老年人行上腹部开放手术术后早期肺不张和氧合的影响，强调呼吸锻炼的时机对术后肺不张的影响。  

Objectives of Study：

1. To evaluate the effect of breathing exercises immediately after extubation in the PACU on the incidence of postoperative pulmonary complications in intermediate or high risk patients undergoing open upper abdominal surgery. 2. To assess the effect of breathing exercises immediately after extubation in the PACU on lung aeration and oxygenation at PACU discharge in patients undergoing open upper abdominal surgery.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

Inclusion criteria

排除标准：

1. BMI ≥ 30kg/m2； 2. 怀孕； 3. 严重的肾脏疾病（肾小球滤过率＜50 ml/min）； 4. 严重的肝脏疾病（Child-Pugh B或C级）； 5. 心肌梗死、心力衰竭和呼吸衰竭病史； 6. 阻塞性睡眠呼吸暂停低通气综合征（OSAHS）； 7. 自发性气胸病史； 8. 神经肌肉疾病； 9. 1个月内的呼吸道感染； 10. 认知功能障碍或其他原因导致无法配合呼吸锻炼； 11. 硬膜外阻滞禁忌证； 12. 未获得知情同意。

Exclusion criteria：

1. BMI ≥ 30kg/m2; 2. Pregnancy; 3. Severe kidney disease (glomerular filtration rate < 50 mL /min); 4. Severe liver disease (Child-Pugh B or C); 5. History of myocardial infarction, heart failure, and respiratory failure; 6. Obstructive sleep apnea-hypopnea syndrome (OSAHS); 7. History of spontaneous pneumothorax; 8. Neuromuscular diseases; 9. Respiratory infections within 1 month; 10. Cognitive dysfunction or failure to understand verbal instructions and cooperate with the breathing exercise 11. Contraindications of epidural block; 12. Failure to obtain informed consent.

本研究采用中心分层随机的方法，统计学家通过SAS系统产生的随机数进行随机分组。 以参与的医院为分层因素，由统计学家按照计算机生成的随机表，按1：1的比例，参与者将被随机分配到对照组或干预组。

Participants will be randomly assigned to either the control group or the intervention group with a 1:1 allocation as per a computer-generated randomization schedule stratified by site.

本研究为单盲设计。 患者本人对分组知情；术前负责教育和呼吸锻炼培训的研究人员对试验分组知情，改研究人员将不再参与后续麻醉、护理及结局评估。 参与麻醉过程的麻醉医生以及外科医生团队、病房护士、参与受试者招募的研究人员和结局指标评估的研究人员对试验分组均不知情。

Blinding：

The participants, as well as the investigator in charge of breathing exercise coaching, will not be blinded. Treatment allocation will be concealed to anesthesiologists, surgeons, nurses, ward staff, and outcome assessors throughout the study.
