研究目的：

阐明在全身麻醉下行髋部骨折修复术的老年患者，术前多次行经鼻注射胰岛素，是否能减少此类人群术后认知功能障碍发生率，降低促炎症因子水平，为临床防治POCD提供新的措施。  

Objectives of Study：

To clarify whether repeated nasal injection of insulin before surgery can reduce the incidence of postoperative cognitive dysfunction in elderly patients undergoing hip fracture repair under general anesthesia, reduce the level of pro-inflammatory factors, and provide new measures for the clinical prevention and treatment of POCD.

纳入标准：

Inclusion criteria

排除标准：

1.有精神疾病史或精神疾病家族史；
2.长期服用精神药物、慢性镇痛药物、皮质类固醇等病史或有酒精滥用史；
3.术前简易智力量表（Mini Mental State Examination, MMSE）评分＜15分；
4.严重的听觉、视觉、语言系统功能损害者；
5.严重肝、肾功能不全；
6.未经治疗或者治疗不足的甲状腺功能亢进患者；
7.不能接受全麻或者对麻醉药物过敏的患者。

Exclusion criteria：

1.Patients with a history of mental illness or family history of mental illness;
2.A history of long-term use of psychotropic drugs, chronic analgesics, corticosteroids, or alcohol abuse;
3.The score of preoperative Mini Mental State Examination (MMSE) was less than 15 points;
4.Severe impairment of hearing, vision and language functions;
5.Severe hepatic and renal insufficiency;
6.Patients with untreated or undertreated hyperthyroidism;
7.Patients who cannot receive general anesthesia or are allergic to anesthetic drugs.

使用计算机SPASS软件完全随机化分组，按I组和C组1:1的比例产生随机数字及随机编号。I组使用胰岛素20IU，C组使用生理盐水2ml。
