研究目的：

血浆置换技术是肝衰竭患者人工肝治疗的重要方式之一。肝衰竭患者常常存在凝血功能障碍，肝素抗凝出血风险高；但无抗凝策略又往往引起管路和血浆分离器堵塞，引起治疗中断。局部枸橼酸抗凝适用于出血风险增高的患者，但缺点是可能引起离子紊乱和枸橼酸蓄积等代谢并发症，有可能加重肝性脑病。 我们设计了一种改良血浆置换枸橼酸抗凝方案，有助于减少4%枸橼酸钠的用量，从而最大限度地减少枸橼酸的并发症。本研究拟对改良血浆置换枸橼酸抗凝方案的效果和安全性进行研究，为人工肝的枸橼酸抗凝提供新的思路和方法。  

Objectives of Study：

Plasma exchange is one of the important methods of artificial liver therapy for patients with liver failure. Patients with liver failure often have coagulation dysfunction and high risk of heparin anticoagulation bleeding, but without anticoagulation strategy, the pipeline and plasma separator are often blocked, resulting in treatment interruption. Local citrate anticoagulation is suitable for patients with increased risk of bleeding, but the disadvantage is that it may cause metabolic complications such as ion disorder and citrate accumulation, and may aggravate hepatic encephalopathy. We designed a modified citrate anticoagulation protocol for plasma exchange, which can help to reduce the dosage of 4% sodium citrate and minimize the complications of citrate. The purpose of this study is to study the effect and safety of modified plasma exchange citrate anticoagulation protocol, and to provide a new idea and method for citrate anticoagulation of artificial liver.

纳入标准：

Inclusion criteria

排除标准：

(1)对血浆过敏者；
(2)因循环不稳定等各种意外中断治疗者；
(3)乳酸>7的高乳酸血症患者；
(4)pH<7.2的顽固酸中毒患者。

Exclusion criteria：

(1) Allergic to plasma;
(2) Due to circulatory instability and other unexpected interruption of treatment;
(3) Patients with lactic acid > 7;
(4) Refractory acidosis patients with pH < 7.2.
