研究目的：

脑卒中后前庭功能障碍以眩晕和头晕、前庭-视觉症状、平衡障碍、姿势步态异常为主要临床表现。部分患者虽经治疗仍会反复眩晕发作、存在不稳感、易跌倒、反复求医，严重影响患者的日常生活、工作及心理，使生活质量明显下降。前庭康复是采用非药物、非创伤手段干预前庭功能障碍的治疗性训练。可用于外周前庭疾病的症状改善和功能恢复，对于急性脑卒中致中枢性前庭功能障碍的疗效尚未达成共识。已有研究表明，脑干梗死后前庭功能的恢复伴随着多感觉皮层及皮层下结构的改变。据此推测，多感觉前庭网络在前庭康复的中枢代偿机制中起关键作用。本研究拟阐述前庭康复在中枢前庭障碍的疗效，并采用神经功能影像学的方法，分析前庭康复治疗前后多感觉前庭皮层、海马、小脑、视觉皮层等皮层及皮层下结构的变化，阐明作用机制，为前庭康复在脑卒中康复中的临床应用提供依据。  

Objectives of Study：

Patients with acute central vestibular syndrome suffer from vertigo, spontaneous nystagmus, postural instability with lateral falls. Vestibular rehabilitation is an exercise program patient-centred that includes a combination of different exercise components with the aim to promote gaze stability, improve balance and gait, and facilitate somatosensory integration. Recent reviews report evidence to support the use of vestibular rehabilitation in people with unilateral peripheral vestibular disorders and with bilateral vestibular loss, for supporting balance and gaze stability training. Despite this evidence, no studies have considered the use of vestibular rehabilitation in patients with central vestibular dysfunction in acute stroke. One prior study found that restitution of vestibular function following brain stem infarcts was accompanied by downstream structural changes in multisensory cortical areas. However, the mechanisms of compensation in vestibular rehabilitation are yet unclear. In this study, we investigate the role of vestibular rehabilitation in patients with central vestibular dysfunction in acute stroke. Specifically, we use MRI images to detect volume and shape differences associated with variation in vestibular function. We aims to explain the mechanism of the broader network of cortical and subcortical structures that receive vestibular input in vestibular compensation, so as to provide theoretical for development of vestibular rehabilitation.

纳入标准：

Inclusion criteria

排除标准：

①双侧或者多灶脑梗死；
②耳源性、眼源性及其他原因导致的前庭功能障碍；
③严重患侧忽略，近 1 个月内有听力突然下降或听力波动性变化者；
④既往有脊髓损伤、截肢、严重下肢关节疾病、骨关节炎或长短腿或骨盆倾斜等影响步行者；
⑤合并心、肺、肝、肾等重要脏器功能衰竭或患有严重基础性疾病。

Exclusion criteria：

1. Bilateral or multifocal infarcts;
2. vestibular dysfunction caused by otogenic, ophthalmic and other reasons;
3. ever unilateral spatial neglect and impairment hearing in 1 month;
4. patients with spinal cord injury, amputation, severe lower extremity joint disease, osteoarthritis, or long and short legs or pelvic tilt that affect walking;
5. presence of neurological, orthopedic or cardiac comorbidities (all of them clinically evaluated).
