研究目的: |
截至目前,国内外对于无痛无汗症的研究,主要集中在病例报道、外周感觉测试,以及免疫、运动及神经方面的血生化检测,而未涉及对其机体脑结构与功能变化影响的研究。而大脑又是作为机体痛觉、情绪产生、处理的核心区域,是疼痛信号产生和传导研究中不可忽视的部分。 功能磁共振技术(functional magnetic resonance imaging,fMRI)在机体接受伤害性刺激时,可对激活的脑部相应的功能区进行成像,进而开展脑功能研究。英国Reading大学科研团队应用fMRI研究了疼痛在大脑中的激活部位,比较了急性疼痛和慢性疼痛的不同,证实了fMRI可以作为疼痛客观评估的新方法;研究团队后来对慢性疼痛的fMRI研究发现慢性疼痛发生时前额叶内侧与伏隔核的联系增强,精度可达80%。可见,fMRI有助于研究疼痛信号处理过程中的脑结构和功能的异常。 综上所述,我们提出如下假说:TrkA通过:1)影响外周伤害性感觉神经元结构和功能,2)影响中枢神经系统痛觉相关脑区结构,进而引发痛觉信号产生、传导及处理异常,导致无痛。 |

Objectives of Study: |
Up to now, the domestic and foreign researches on congenital insensitivity to pain with anhidrosis mainly focus on case reports,peripheral sensory tests, and blood biochemical tests in immune, motor and nerve aspects, but do not involve the studies on the effects on the brain structure and function of the body. As the core area of pain perception, emotion generation and processing, the brain is an important part in the research of pain signal generation and transmission.Functional magnetic resonance imaging (fMRI) can image the corresponding functional areas of the activated brain when the body receives traumatic stimulation, and then carry out the study of brain function.A team from the University of Reading in the UK used fMRI to study the activation of pain in the brain, comparing the differences between acute and chronic pain, and confirming that fMRI could be a new method for objective assessment of pain. The team later performed fMRI studies of chronic pain and found that the medial prefrontal lobe strengthened its connection to the nucleus accumbens with an accuracy of 80 percent during chronic pain. Therefore, fMRI is helpful to study the abnormalities of brain structure and function during pain signal processing.To sum up, we propose the following hypothesis: Trka affects the structure and function of peripheral nociceptive sensory neurons, and 2) affects the structure of pain-related brain regions of the central nervous system, and then causes abnormal production, transmission and processing of pain signals, leading to painlessness. |

有慢性疼痛疾病史、酗烟酒史、近期服用过镇痛药物史、神经精神疾病病史及其他磁共振检查禁忌者。 |

Patients with a history of chronic pain disease, history of alcohol and tobacco abuse, recent use of analgesic drugs, history of neuropsychiatric diseases and other contraindications to MRI examination. |

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture: |
1) The Data Management Committee is composed of the Clinical Trial Ethics Committee of Tongji Medical College of Huazhong University of Science and Technology, key researchers in the Department of Anesthesiology of Tongji Hospital Affiliated to Huazhong University of Science and Technology and the Clinical Trial Registry; 2) Select Resman original data sharing platform for database; 3) According to the experimental needs, the researcher independently designed and filled in the unified CRF form in RESMAN; 4) Inspectors check and transfer to the data management unit; 5) Establish input procedure according to CRF; 6) Double and independent input; 7) Verify and generate DQF; 8) The supervisor will return the DQF to the researcher for review and answer; 9) Database modification and verification; 10) Data review in the blind state; 11) Data locking; 12) Data transmission (statistical analysts). |
