研究目的:

血小板经由表现CD40L及TLR4而成为发炎的部分核心。输血反应的本质是发炎反应,而输血反应对于CD40L及TLR4的表现仍未知。因此我们设计此实验来验证在输血过程中,ABO血型吻合下的血小板在CD40L和TLR4的表现情况是否优于ABO血型不吻合下的血小板,富含血小板之血桨(platelet rich plasma )再去除血桨,即可得到洗涤血小板(washed platelet),此去除之血桨,亦可能对血小板功能产生抑制之影响,故可能临床上输washed platelet并不一定会优于输platelet rich plasma之血小板。因此,本实验于体外模拟输血观察CD40L及TLR4之表现,并探讨其可能的机转,亦观察ABO相同或不同之模拟输血对于CD40L及TLR4的表现影响并探讨可能引起发炎反应的机制。

Description for medicine or protocol of treatment in detail:

In vitro blood mixtue were divided into three groups We add donor blood into recipient blood to a portion of 1%, 5%, 10% volume/volume mixture to simulate in vitro transfusion. In vitro blood transfusions were divided into three groups (Group M, Group S, and Group I). 1.Group M consisted of donor cross-matched blood mixed with recipient whole blood; the cross-matched blood procedure mimics transfusion. Cross-matching serves three functions: (1) it confirms ABO and Rh typing, (2) it detects antibodies against other blood group systems, and (3) it detects low-titer antibodies or those that do not easily agglutinate. The other two groups did not involve cross-matching and thus do not provide these three functions. 2.Group S consisted of donor ABO type-specific uncross-matched blood mixed with recipient whole blood (the same blood type but not cross-matched) 3.Group I involved donor ABO incompatibility blood mixed with recipient whole blood (not ABO type-specific blood and not process cross-matched).
