研究目的：

肩关节镜手术是目前临床上常见的手术类型之一，然而术后中度到重度的疼痛一直是难以解决的问题。文献报道，术后严重疼痛可导致焦虑、睡眠质量下降，术后认知功能低下，甚至影响患者的预后，延长住院时间，对家庭和社会造成严重的负担，违背了加速康复外科的理念。传统的术后镇痛方式主要为静脉镇痛药物，静脉镇痛泵给药以及硬膜外镇痛。然后静脉镇痛药物的副作用（胃肠道反应），镇痛泵阿片类所致的不良反应（尿储留、呼吸抑制、恶心呕吐等），硬膜外镇痛的局限性（穿刺困难、对凝血功能要求高、瘙痒）等都限制了其在临床上的普遍性。超声引导下竖脊肌平面阻滞（ESPB）最早由国外学者于2016年首次提出并应用于胸椎区域慢性疼痛的治疗。几年来，关于ESPB的机理充满了争议。有文献报道尸体解剖发现胸段竖脊肌平面注射燃料后，并不能扩散到椎旁间隙进而影响到脊神经腹侧支。然而，另有学者有不同的观点，目前文献报道竖脊肌平面阻滞在乳腺癌根治术后的镇痛发挥了重要作用。国内研究人员认为竖脊肌平面阻滞效果因人而异，有学者提出了“屋顶理论”和“渗透理论”。然而，目前关于ESPB的效果依旧争议不断。本研究主要通过超声引导下对肩关节手术患者进行ESPB，观察其术后镇痛的效果。  

Objectives of Study：

Shoulder arthroscopic surgery is one of the most common surgical types in clinical practice. However, moderate to severe postoperative pain has always been a difficult problem to solve. It is reported that severe postoperative pain can lead to anxiety, decreased sleep quality, poor postoperative cognitive function, and even affect the patient's prognosis and prolong the hospital stay It imposes a severe burden on the family and society and goes against the concept of accelerated rehabilitation surgery.Traditional methods of postoperative analgesia are mainly intravenous analgesics, intravenous analgesia pump administration and epidural analgesia.Then the side effects of intravenous analgesic drugs (gastrointestinal reactions), analgesic pumps caused by opioids are adverse reactions (urine retention, respiratory depression, nausea and vomiting, etc.), limitations of epidural analgesia (difficult puncture, high requirement for coagulation function, pruritus, etc.) has limited its clinical universality. Ultrasound-guided erector spinal plane block (ESPB) was first proposed by foreign scholars in 2016 and applied to the treatment of chronic pain in the thoracic vertebral region.Over the years, the mechanism of the ESPB has been the subject of much debate.It has been reported in the literature that autopsy has found that the thoracic erector spinal muscle does not diffuse after plane injection of fuelTo the paravertebral space which then affects the ventral ramus of the spinal nerve.However, other scholars have a different view, and the current literature reports erector spinal plane block in the mammary gland. Analgesia after radical resection of cancer plays an important role.Domestic researchers believe that the erector spinal plane block effect varies from person to person, some scholars have proposed "roof Theory "and" osmosis theory ".However, the effectiveness of the ESPB remains controversial.In this study, the hand of shoulder joint was guided by ultrasoundThe postoperative analgesic effect of ESPB was observed.

纳入标准：

Inclusion criteria

排除标准：

1、患者有严重的术前合并症，如呼吸系统、心血管系统等；
2、凝血功能障碍；
3、长期使用阿片类药物；
4、肥胖（BMI≥35）；
5、穿刺点皮肤感染。

Exclusion criteria：

1. The patient has serious preoperative complications, such as respiratory system and cardiovascular system;
2, Coagulation dysfunction;
3. Long-term use of opioids;
4. Obesity (BMI >= 35);
5. Skin infection at the puncture site.
