研究目的：

初步评价吡非尼酮预防不可切除局部晚期食管癌放疗患者放射性肺损伤的有效性和安全性。  

Objectives of Study：

Objective to evaluate the efficacy and safety of pirfenidone in preventing radiation-induced lung injury in unresectable patients with esophageal cancer undergoing chemoradiotherapy.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

Inclusion criteria

排除标准：

1. 既往接受过胸部放射治疗史；
2. 同时合并其他部位恶性肿瘤受试者（非黑色素的皮肤癌、原位宫颈癌或治愈的早期前列腺癌除外）；
3. 凝血功能异常、具有出血倾向（如活动性消化道溃疡）或正在接受溶栓或抗凝治疗；
4. 有间质性肺病病史、非感染性肺炎病史
5. 合并活动性结缔组织病，尤其硬皮病和系统性红斑狼疮或胶原血管疾病者；
6. 合并严重的心肺疾病（充血性心力衰竭、不能控制的高危性心律失常、不稳定性心绞痛、半年内的心肌梗塞、重度心瓣膜疾病以及顽固性高血压等）、未控制的感染性疾病等其他非肿瘤相关疾病，使患者无法耐受放化疗等综合治疗；
7. 有精神疾病或其他原因无法签署知情同意书；
8. 医从性差，研究组认为患者无法按标准完成试验治疗流程及后续随访。

Exclusion criteria：

1. Patients who have received chest radiotherapy in the past;
2. Patients with other malignant tumors (except for non melanin skin cancer, cervical cancer in situ or cured early prostate cancer);
3. Patients with abnormal coagulation function, bleeding tendency (such as active peptic ulcer) or receiving thrombolytic or anticoagulant therapy;
4. Patients with history of interstitial lung disease and non infectious pneumonia;
5. Patients with active connective tissue disease, especially scleroderma, systemic lupus erythematosus or collagen vascular disease;
6. Patients with severe cardiopulmonary diseases (congestive heart failure, uncontrollable high-risk arrhythmia, unstable angina pectoris, myocardial infarction within half a year, severe heart valve disease and intractable hypertension, etc.), uncontrolled infectious diseases and other non tumor related diseases make the patients unable to tolerate comprehensive treatment such as radiotherapy and chemotherapy;
7. The subjects with mental illness or other reasons can not sign the informed consent;
8. For the subjects with poor medical compliance, the study group thought that the patients could not complete the trial treatment process and follow-up according to the standard.
