研究目的：

观察高碳酸血症对非气管插管全麻胸腔镜手术患者术后认知功能的影响。通过运用局部脑氧饱和度( rScO2)监测手段指导临床PETCO2的调控，以增加脑氧供，降低神经系统损伤的发生率,为临床工作提供参考。  

Objectives of Study：

To observe the effect of hypercapnia on cognitive function in patients undergoing non-intubated spontaneous-ventilation video-assisted thoracoscopic surgery.Through the use of local cerebral oxygen saturation (rsCO2) monitoring method to guide the clinical regulation of PETCO2, in order to increase cerebral oxygen supply and reduce the incidence of nervous system injury,to provide reference for clinical work.

纳入标准：

Inclusion criteria

排除标准：

①拒绝手术或麻醉方案，既往胸腔镜手术史；
②中-重度慢性阻塞性肺疾病 （COPD），术前低氧血症(PaO2<60 mmHg)或高碳酸血症 (PaCO2>50mmHg)，严重急性肺部感染
③术前检查提示心律失常，心功能不全；
④局麻药过敏，凝血功能障碍
⑤体重指数（BMI）>28 kg/m2；
⑥广泛胸膜粘连或合并复合肺叶和或肺段切除；
⑦中转插双腔气管导管或中转开胸；
⑧合并神经精神疾病或其他重要器官功能不全。

Exclusion criteria：

1. The subjects who refused the operation or anesthesia plan had a history of thoracoscopic surgery;
2. Patients with moderate to severe chronic obstructive pulmonary disease (COPD) had preoperative hypoxemia (PaO2 < 60 mmHg) or hypercapnia (PaCO2 > 50 mmHg) and severe acute pulmonary infection;
3. Patients with arrhythmia indicated by preoperative examination have cardiac insufficiency;
4. Coagulation dysfunction in patients with local anesthetic allergy;
5. Subjects with body mass index (BMI) > 28 kg / m2;
6. Patients with extensive pleural adhesion or complicated lobectomy or segmental resection;
7. Patients transferred to double lumen endotracheal tube or thoracotomy;
8. Patients with neuropsychiatric diseases or other important organ dysfunction.
