研究目的：

评估聚丙烯缝线辅助的近360°小梁切开术治疗儿童青光眼的手术效果及安全性不差于光纤微导管辅助的360°小梁切开术，主要包括降眼压效果、术中术后并发症。

Objectives of Study：

To evaluate the effect and safety of prolene-assisted nealy 360 trabeculotomy not inferior to microcatheter-assisted 360 trabeculotomy

药物成份或治疗方案详述：

所有符合入组条件的儿童青光眼患儿分为两组，分别接受聚丙烯缝线辅助及光纤微导管辅助的近360°小梁切开术。主要手术步骤：沿上方角膜缘剪开球结膜，制作约1/2巩膜厚度、4×4 mm大小的巩膜瓣，在浅层巩膜瓣内侧平行于浅层巩膜瓣切口制作约2x3mm大小的深层巩膜瓣，小心剖切并打开Schlemm管外壁，剪除深层巩膜瓣，小心剖切形成小梁网-狄氏膜（TDM）。把5/0聚丙烯缝线或光纤微导管穿入Schlemm管一侧断端，顺Schlemm管走行全周，直至从对侧断端穿出。用镊子夹住微导管或聚丙烯线的两端，牵拉切开Schlemm管内壁和小梁网。10-0尼龙线间断缝合巩膜瓣，冲洗并形成前房，缝合球结膜切口。如果角膜透明可选择房角镜辅助下内路穿管和切开：做上方透明角膜切口，向前房注入粘弹剂，做右下方角膜缘侧切口，置入微导管或聚丙烯缝线备用。房角镜下观察小梁网，用1mm注射器针头划开下方小梁网及Schlemm管内壁约1-2个钟点，将微导管或聚丙烯缝线置入Schlemm管，沿管穿行全周，自对侧断端穿出，用镊子夹住微导管或聚丙烯线的两端，牵拉切开Schlemm管内壁和小梁网。冲洗并形成前房，水密角膜切口。

Description for medicine or protocol of treatment in detail：

The enrolled patients will be divided into two groups, prolene group and microcatheter group.The procedures are listed as follow: The sclera was liberated from conjunctiva and Tenons capsule. Then, a superficial scleral flap of 4 x 4 mm and half thickness was dissected and lifted towards the limbus. Underneath the first flap, a deep one of 2 × 3 mm was created uncovering the choroid completely. It was then lifted in the limbal direction until the scleral spur was reached. Then, the sclera and scleral spur were carefully separated, and Schlemms canal was accurately localized behind the scleral spur. A trabecular-Descemets membrane (TDM) was created. Schlemms canal was unroofed and two ostia were created. The tip was inserted into one ostium of Schlemms canal. The microsurgical forceps were used to push the suture through the entire canal. The tip came out from the other ostium through nearly 360-degree advancement. Traction was placed on the proximal aspect of the 5/0 suture, thus creating a nearly 360-degree ab externo trabeculotomy except the part of the canal in the deep scleral flap. The superficial scleral flap and conjunctiva flap were sutured with 10/0 nylon suture. Gonioscopy-assisted transluminal trabeculotomy will be performed if the cornea is transparent. The procedure is performed via micro-incisions in the cornea. An incision about 1 mm in size is made in the periphery of the cornea. After entering the eye, the surgical procedure involves cutting through the trabecular meshwork, cannulating Schlemms canal 360°and unroofing Schlemm's canal.

纳入标准：

Inclusion criteria

排除标准：

1.合并其他眼部疾病；
2.曾行青光眼手术以外的其他眼部手术；
3.合并没有得到控制的全身疾病；
4.不能配合复查和随访。

Exclusion criteria：

1. complicated with other congenital eye anomalies;
2. with a history of eye surgeries other than glaucoma surgery;
3. complicated with uncontrolled systemic diseases;
4. could not finish the whole-course follow-up.
