研究目的：

丙泊酚复合舒芬太尼用于无痛人流术已在临床普及应用，但术中体动发生率高，对呼吸、循环系统有抑制作用。右美托咪定是一种相对选择性α2-肾上腺素受体激动剂，具有镇静、镇痛、抗焦虑作用，且对呼吸无抑制作用。右美托咪定与丙泊酚联合使用能产生良好的镇静和镇痛作用，可降低静脉麻醉药的用量及呼吸抑制、恶心呕吐及头昏等不良反应发生率。已有研究表明：全麻诱导前应用1ug/kg盐酸右美托咪定滴鼻可有效改善小儿术前焦虑情绪，降低小儿麻醉苏醒期躁动的发生率。但右美托咪定鼻滴应用于无痛人流术的临床效果尚不明确。本文旨在观察右美托咪定在人流术前鼻滴对患者解焦、血流动力学、异丙酚用量以及患者和医生的满意度的影响。

Objectives of Study：

In China, artificial abortion-vacuum aspiration is a short-time surgery in the clinic of gynaecology andobstetrics andit may cause pain, anxiety andnervousness perioperatively. Deep sedation without intubation is a feasible method of anesthesia for first trimester surgical abortions in the outpatient setting. Propofol andcombination of propofol andsulfentanil are widely used for short-time deep sedation in China. But they may cause respiratory depression, hypoxemia, pain of propofol injection, postoperative nausea andvomiting andso on. Dexmedetomidine is an α2-adrenorecepter agonist with high selectivity of alpha-2 to alpha-1, 1620 to 1which is eight times that of clonidine andit provides sedation, anxiolysis andanalgesia. Intranasal dexemdetomidine has showed effective andwell tolerated with reliable sedation without effect on pain pressure threshold in healthy volunteers. Many studies showed intranasal dexmedetomidine has been proved to offer good sedative effect in childrens premedication, andsedative andanalgesic effect in adults surgery such as third molar extraction without unwanted respiratory depression or bradycardia. To date, there is no study has been done on whether intranasal dexmedetomidine may offer sedative, analgesic andanxiolytic effect in artificial abortion-vacuum aspiration. We designed this double-blinded, randomized, controlled study to evaluate sedative, analgesic andanxiolytic efficacy of intranasal dexmedetomidine in artificial abortion-vacuum aspiration in China.

Inclusion criteria

排除标准：

有明显心、肺、肾疾病史及药物过敏史，有流产史或者生育史，对右美托咪定、异丙酚、舒芬太尼过敏，哮喘病史、精神疾病、呼吸睡眠暂停综合征，体重指数>24kg/m2,嗜酒。

Exclusion criteria：

Patients with known allergy or regular use of dexmedetomidine, opoids andpropofol, sedatives, andhistory of abortion or labor, impaired liver or renal function, alcohol consumption in excess 28 units per week, known psychological disorder, asthma, heart disease, BMI more than 24kg/m2, sleep apnea syndrome andpatient refusal were excluded
