研究目的：

本项目研究以随机平行对照为设计原则，以切开挂线术组为对照组，以括约肌间切开术为试验组，以肛门括约功能、术后复发率、疼痛指数、创面面积等为观察指标，以肛瘘专项数据库为信息平台，通过系统、深入地研究括约肌间感染机制指导下的括约肌间切开术治疗高位肛瘘对肛门括约功能、疼痛指数、创面愈合时间、复发率、创面面积、住院天数、术后肛门外观形态等方面的影响，论证括约肌间切开术治疗高位肛瘘与切开挂线术相比在复发率、肛门括约功能保护、局部疼痛、创面愈合等方面的优势。 评估基于括约肌间感染机制的括约肌间切开术治疗高位肛瘘的有效性、安全性及与传统切开挂线术相比的优越性，产生治疗高位肛瘘的新技术和新观点，从而推广这一技术应用于高位肛瘘临床实践。  

Objectives of Study：

In this study, random parallel control was used as the design principle, incision and hanging suture group as the control group, intersphincterotomy as the experimental group, anal sphincter function, postoperative recurrence rate, pain index, wound area and so on as the observation index. take the special database of anal fistula as the information platform. Through a systematic and in-depth study of the effect of intersphincterotomy on anal sphincter function, pain index, wound healing time, recurrence rate, wound area, hospitalization days and postoperative anal appearance under the guidance of intersphincter infection mechanism. To demonstrate the advantages of intersphincterotomy in the treatment of high anal fistula in terms of recurrence rate, protection of anal sphincter function, local pain, wound healing and so on. To evaluate the efficacy, safety and superiority of intersphincterotomy based on the mechanism of intersphincter infection in the treatment of high anal fistula, and to produce new techniques and viewpoints in the treatment of high anal fistula. in order to promote the application of this technique to the clinical practice of high anal fistula.

纳入标准：

Inclusion criteria

排除标准：

①合并有肿瘤、结核、糖尿病、营养不良、炎症性肠病、梅毒、艾滋病等影响切口生长或愈合的相关性疾病的患者；
②患有严重的重要脏器疾病和/或精神异常的患者；
③妊娠、哺乳期及月经期妇女；
④既往有肛瘘手术史或瘢痕体质的患者。

Exclusion criteria：

1.Patients with tumors, tuberculosis, diabetes, malnutrition, inflammatory bowel disease, syphilis, AIDS and other related diseases that affect the growth or healing of incisions.
2.patients with severe important organ diseases and / or mental disorders.
3.pregnant, lactation and menstrual women.
4.patients with previous operation history of anal fistula or scar constitution.
