研究目的：

在重度牙周炎患牙和牙列的牙周再生中通过保留肉芽组织的治疗方式，探索利用牙周内窥镜辅助下的有效清创，在不翻瓣的情况下获得牙周再生的有利空间和血供，联合植骨材料或生长因子的应用，并采用激光辅助杀菌和生物学刺激调节牙周局部微环境，龈下喷砂获得维护期的有效菌斑控制，本课题的重点在于评估上述方案对重度牙周炎患牙和牙列保存治疗的可行性，为重度牙周炎患牙和牙列的保存性治疗提供新的思路。  

Objectives of Study：

In the treatment of periodontal regeneration of teeth and dentition with severe periodontitis, effective debridement assisted by periodontal endoscopy was explored to obtain favorable space and blood supply for periodontal regeneration without flap turnover, combined with the application of bone graft materials or growth factors, and laser assisted sterilization and biological stimulation were used to regulate the local periodontal microenvironment, subgingival sandblasting In order to achieve effective plaque control in the maintenance period, the focus of this project is to evaluate the feasibility of the above scheme for the preservation treatment of teeth and dentition with severe periodontitis, and to provide a new idea for the preservation treatment of teeth and dentition with severe periodontitis.

纳入标准：

Inclusion criteria

排除标准：

①　试验牙1年内有牙周开放性清创手术史；
② 已知对动物源胶原过敏者；
③　临床或放射学显示手术部位有急性感染，根尖病变，根折，严重的牙根畸形，牙骨质珠，不易磨除的牙骨质釉质突起，在釉牙骨质界（CEJ）或根面有未经治疗的龋坏，修复体达龈下和/或在CEJ处或其下方修复体边缘不密合；
④　各种能够导致口腔影像检查伪影的受试者，如：研究牙齿及其邻牙为金属义齿、烤瓷牙；
⑤　有糖尿病病史的患者或血糖检测异常者（空腹血糖≥7mmol/L）；
⑥　肝、肾功能异常者（AST、ALT≥1.5倍ULN，肌酐≥1.5倍ULN）；
⑦　有严重的内分泌及代谢性疾病患者；
⑧　有三级高血压病史者；有骨质疏松及自身免疫性疾病病史者；
⑨　因恶性肿瘤或其他严重疾病病史不宜接受手术者；
⑩　知情同意书签署前3个月内或正在使用干扰钙代谢的类固醇药物的患者。

Exclusion criteria：

1. The experimental teeth had a history of open debridement within one year;
2. Known to be allergic to animal derived collagen;
3. Clinical or radiological findings showed that there were acute infection, apical lesions, root fracture, severe root deformity, cementum beads, hard to remove cementum enamel protrusion, untreated caries at the enamel cementum boundary (CEJ) or root surface, and the restoration reached subgingival and / or the edge of restoration at or below CEJ was not close;
4. All kinds of subjects can cause artifacts in oral imaging examination, such as: the study teeth and their adjacent teeth are metal dentures and porcelain fused to metal teeth;
5. Patients with history of diabetes or abnormal blood glucose detection (fasting blood glucose >= 7 mmol / L);
6. Abnormal liver and kidney function (AST, ALT >= 1.5 fold ULN, creatinine >= 1.5 fold ULN);
7. Patients with severe endocrine and metabolic diseases;
8. The patients with three-level hypertension had osteoporosis and autoimmune diseases;
9. Who is not suitable for surgery due to the history of malignant tumor or other serious diseases
10. Patients who were using steroids that interfere with calcium metabolism within 3 months prior to informed consent bookmarking.
