研究目的:

单独使用FES、TDCS或TDCS结合作业训练治疗脑卒中上肢功能障碍的临床报道并不在少数,但可靠的RCT研究很少,而将TDCS与FES治疗结合起来探究这种联合康复干预对慢性期脑卒中患者手功能恢复的脑机制研究的报道则还未见到。因此,本研究尝试采用TDCS治疗首先通过提高运动相关脑区大脑皮层的兴奋性,接着进行FES治疗,在现实生活中强化和训练患者使用瘫痪上肢,旨在探讨这种联合康复疗法对患者上肢功能恢复的影响,且用功能磁共振,经颅磁刺激,表面肌电图以及康复临床量表来对前后中枢神经,外周神经和肌肉的信号进行分析研究。  

Objectives of Study:

Although there already have many researches on TDCS and FES alone or combinging with occupatianal therapy, valid randomized controlled studies are lack and our study is the first the investigate the effect of TDCS combine FES therapy on recovery of hund funciton after stroke. Thus, our purpose is to study this new method namely up-regulate the motor cortex exitability of patients followed by FES therapy to retrain the impaired hand funtion. We also eveluate the central and periphral nerve system and musclar systems' signals using various assessment method and tools such as fMRI, TMS, SEMG and rehabilitaion sacles pre-and-post the intervention.

纳入标准:

Inclusion criteria

排除标准:

(1)诊断不符合1995年全国第四届脑血管病学术会议通过的各类脑血管病诊断要点 (2)非首次发生脑出血和脑梗死或既往发生过腔隙性脑梗死且存在后遗症的患者 (3)意识不清,合并感觉性失语或精神障碍等疾患,不能配合检查及治疗的住院患者 (4)合并心、肝、肾和造血系统等严重原发性疾病的患者 (5)既往有认知障碍、神经精神病史、药物滥用及酗酒史 (6)有重要脏器功能衰竭或病情危重的脑卒中患者以及癌症等对患者生存质量的影响超过中风后肢体功能障碍的疾病或状态(7)距发病时间<6个月 (8)不能完成基本疗程,依从性可能不好者(即不能坚持治疗)及难以随访者 (9)上肢有金属植入物者或颅骨缺损者(10)有幽闭恐惧症等,无法接受fMRI检查者

Exclusion criteria:

(1) Not satisfying the cerebrovascular disease diagnostic criteria defined by The Fourth National Conference on Cerebrovascular Disease, 1995;
(2) Not first cerebral hemorrhage or cerebral infraction, lacunar infraction with squelae;
(3) Unconsciousness,or with sensory aphasia or mental disorders, cannot coordinate examination and treatment;
(4) With serious illnesses, such as heart, liver or kidney diseases, or serious coagulation disorders;
(5) With a history of cognitive disorder, neuropsychiatric disorder, drug or alcohol abuse;
(6) With organ failure, carcinoma or fatal stroke that seriously affect life quality more than hand dysfunction;
(7) Time since stroke less than 6 months;
(8) Cannot complete basic course, cannot persist treatment, or be difficult to follow-up;
(9) With upper limb metal implants or skull defect;
(10) Claustrophobia, unable to accept the fMRI examination.
