研究目的：

1. 管理组糖尿病患者血糖、血压、血脂3项达标率在全国平均基础上，增长50%以上； 2. 糖尿病患者HbA1c检测率达到60%、血脂检测率达到80%、血压检测率基本达到80%； 3. 证明糖尿病专科护士在糖尿病团队管理中起重要作用，SDTM管理模式适合于目前中心医院的糖尿病医疗实践。  

Objectives of Study：

1. To increase the targeting rates of blood glucose,blood pressure and blood lipids; 2. To achive the testing rates of HbA1c, lipids, blood pressure; 3. To prove the importance of diabetic nurses in diabetes management team, and the suitability of SDTM for the current diabetes care model.

纳入标准：

Inclusion criteria

排除标准：

1、意识不清，有认知功能障碍，无法交流；
2、近期诊断癌症或其他威胁生命状态，预期寿命小于2年者；
3、不能参加随访者。

Exclusion criteria：

1. With cognitive dysfunction;
2. Patients with reduced life expectancy less than 24-months due to recent diagnosis of advanced cancers and other life threatening conditions;
3. Patients who are unwilling to return for regular follow up.
