研究目的：

1.通过横断面研究了解广东农村老年人群2型糖尿病控制现状。2.通过病例对照研究，分析探讨导致该人群糖尿病控制不佳的风险因素，并据此构建风险预测模型，为早期识别提供量化风险概率评估理论依据。3.旨在构建农村老年夫妻糖尿病协同远程管理模式，并验证该模式在我国农村老年糖尿病管理中应用的有效性。  

Objectives of Study：

1. To understand the current status of type 2 diabetes control in the elderly in rural areas in Guangdong through cross-sectional research. 2. Through case-control studies, analyze and explore the risk factors that lead to poor diabetes control in this population, and build a risk prediction model accordingly to provide a theoretical basis for quantitative risk probability assessment for early identification. 3. The aim is to build a collaborative remote management model for rural elderly couples diabetes and to verify the effectiveness of this model in the management of rural elderly diabetes in my country.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）达到国家基本公共卫生服务2型糖尿病健康管理规范中要求的确诊2型糖尿病患者；
（2）最近一次空腹血糖水平> 8.0 mmol/L或糖基化血红蛋白> 7.0％或新诊断的T2DM；
（3）年龄≥55岁；
（4）具有基本的读写能力，足够的认知能力和身体能力；
（5）与配偶同住；
（6）自愿意参与研究，知情同意。
符合条件的配偶（1）已婚或与T2DM病人同居； （2）无干扰研究的精神或身体机能障碍； （3）自愿意参与研究，知情同意。

Inclusion criteria

1. confirmed T2DM patients who registered for the National Essential Public Health Services for T2DM Management;
2. with the latest fasting blood glucose level >8.0 mmol/L or the latest glycosylated hemoglobin >7.0% or newly diagnosed T2DM during the past 12 months;
3. aged >=55 years;
4. with basic literacy, adequate cognitive and physical capability;
5. living with spouses;
6. willing to provide informed consent to participate in the study.
Eligible spouses are married or cohabitating with a T2DM patient; (2) without mental or physical dysfunctions that may interfere with the study; (3) willing to provide informed consent.

排除标准：

(1) 以前参加过T2DM教育课程的患者将被排除在外；
(2) 为了进一步区分患者和配偶，我们进一步排除了患有糖尿病的夫妇。

Exclusion criteria：

1. Patients who previously participated in T2DM education courses;
2. We further exclude couples that both have diabetes, for a clear distinction between patients and spouses.

不同社区健康服务中心的每一位参与者将会被以1:1的比例随机分配至夫妻组和个人组。统计分析人员将会按患者性别和年龄组进行分层随机化，同时进行层序随机化。

Participants of each community health center will be randomized 1:1 within center into a couple-based arm and an individual-based arm. Stratified randomization by patients’gender and age group will be performed alongside randomization of the class sequence,by a statistician otherwise not involved
