研究目的：

1>找到膈顺指数预测患者撤机的cut off值 2>检验其与目前常用2个指标（D-RSBI、DTF）在预测患者撤机过程中灵敏度、特异度、阳性预测值、阴性预测值之间的差别  

Objectives of Study：

1>to find the cut off index of the diaphragm-compliance index 2>to text the different of diaphragm-compliance index ,DTF and D-RSBI in sensitivity, specificity, positive predictive value and negative predictive value

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）机械通气时间超过48小时；
（2）导致机械通气的原发病改善或恢复；
（3）氧合稳定（氧合指数≥200mmHg，PEEP≤5cmH2O，吸入氧浓度＜50%）；
（4）血流动力学稳定，无心肌缺血的动态表现，不需要或仅需要小剂量血管活性药物维持（多巴胺＜5ug/kg·min或去甲肾上腺素＜0.05ug/kg·min）；
（5）有自主咳痰能力

Inclusion criteria

(1) Mechanical ventilation time exceeds 48 hours; (2) leads to the primary disease to improve or restore mechanical ventilation; (3) oxygenation stability (oxygenation index 200 MMHG, or PEEP 5 or less cmh2o, inhaled oxygen concentration < 50%). (4) hemodynamic stability, the dynamic performance of myocardial ischemia, need not or only need small doses of asoactie drugs to maintain dopamine (< 5 ug/kg min or norepinephrine < 0.05 ug/kg, min); (5) has a capacity for independent sputum

排除标准：

（1）既往肺部肿瘤、肺切除、胸壁畸形导致呼吸力学异常等；
（2）研究前48h内使用肌松剂；
（3）有膈肌麻痹病史者；
（4）神经肌肉疾病患者

Exclusion criteria：

(1) Abnormal respiratory mechanics caused by previous lung tumor, lung resection, chest wall deformity, etc.; (2) research within 48 h before use nondepolarizing agent; (3) a history of diaphragmatic myoparalysis; (4)patients with neuromuscular disease
