研究目的：

本研究将开展随机、对照临床试验（RCT），比较在获得血培养或其他体液培养微生物的结果之前，进行“纳米孔测序指导的抗生素治疗”vs“经验性抗生素治疗”对患者28天脓毒症休克及死亡复合终点的影响。  

Objectives of Study：

This study will conduct a randomized, controlled clinical trial (RCT) to compare the impact of "nanopore sequencing guided antibiotic therapy" vs "empirical antibiotic therapy" on the composite endpoint (septic shock or death in 28 day) of sepsis patients before obtaining the results of blood culture or other body fluid culture microorganisms.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

疑似脓毒症患者，符合以下①项加②~④项中任意一项：①感染或疑似感染患者；②qSOFA≥2；③SOFA=1；④NEWS评分4~6分。
脓毒症患者：符合第3版脓毒症及脓毒症休克国际共识定义（Sepsis-3）的脓毒症或脓毒症休克的临床诊断标准。

Inclusion criteria

Patients with suspected sepsis should meet ① plus any of ②~④: ① patients with infection or suspected infection; ②qSOFA≥2 ③SOFA=1 ④ News score was 4-6.
Patients with sepsis: Patients should meet clinical diagnostic criteria of sepsis or septic shock according to sepsis-3.

排除标准：

①年龄＜18岁；②患有自身免疫性疾病、获得性免疫缺陷综合征、粒细胞缺乏、恶性肿瘤或其他严重慢性疾病（心力衰竭、肝衰竭、终末期肾病等）；③正在接受糖皮质激素治疗；④妊娠；⑤拒绝入组或放弃积极治疗。

Exclusion criteria：

① Age < 18 years old; ② Patients with autoimmune diseases, acquired immunodeficiency syndrome, agranulocytosis, malignant tumor or other serious chronic diseases (heart failure, liver failure, end-stage renal disease, etc.); ③ Receiving glucocorticoid treatment; ④ Pregnancy; ⑤ Refuse to be enrolled or give up active treatment.
