研究目的：

构建心肌MRI多模成像手段，实现对心脏形态功能、心肌形变及水肿、代谢异常、微循环障碍、心肌纤维化及心室重构的“一站式”评价，拟解决如何整合优化这些成像方法，形成高敏感及特异性的多模态MRI成像是需要解决的技术难题。通过临床随访，明确心脏不良事件发生情况；采用多因素Cox风险分析筛选不同心肌损伤、代谢异常及微循环障碍指标在远期心脏不良事件中的独立风险因子，明确其预测价值。最终为临床2019-nCoV患者心肌损伤提供敏感性及特异性强的影像学评价手段，并为早期靶向心脏治疗提供科学依据。  

Objectives of Study：

To construct a multi-mode imaging method of myocardial MRI, to realize the "one-stop" evaluation of cardiac morphology and function, myocardial deformation and edema, metabolic abnormalities, microcirculation disorders, myocardial fibrosis and ventricular remodeling, and to solve the technical problem of how to integrate and optimize these imaging methods to form a highly sensitive and specific multi-mode MRI imaging. Through clinical follow-up, the occurrence of adverse cardiac events was determined; the independent risk factors of different indexes of myocardial injury, metabolic abnormality and microcirculation disturbance in long-term adverse cardiac events were screened by multivariate Cox risk analysis, and the predictive value was determined. Finally, it can provide a sensitive and specific imaging evaluation method for myocardial injury in 2019-ncov patients, and provide a scientific basis for early targeted cardiac therapy.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）根据国家卫健委和中医药管理局对《新型冠状病毒感染的肺炎诊疗方案（试行第五版）》确诊的2019-nCoV感染患者。
（2）无MR检查禁忌症；
（3）理解潜在风险/收益，患者或监护人同意参与研究，了解全部研究程序，同意并依从研究随访方案进行随访。

Inclusion criteria

1. novel coronavirus pneumonia diagnosis and treatment plan (trial version fifth) was confirmed by 2019-nCoV and the Chinese medicine authority;
2. No contraindications for MR examination;
3. understand the potential risks / benefits, patients or guardians agree to participate in the study, understand all the study procedures, agree the plan of follow-up.

排除标准：

（1）合并先天性心脏病、原发性及其它继发性心肌病的患者；严重心律不齐、频发房颤，图像质量伪影严重；
（2）MRI 检查禁忌症：体内有铁磁性金属异物（例如心脏起搏器?神经刺激器；人工金属心脏瓣膜；动脉瘤夹；眼内金属异物；内耳植入；金属假体；金属假肢；金属关节等）；详见《增强磁共振检查知情同意书》；
（3）幽闭空间恐惧症者;近期发生寻麻疹、过敏性皮炎等过敏性疾病；
（4）钆造影剂禁忌症：肾小球滤过率估计值（eGFR）＜30 mL/min/1.73㎡;既往注射钆对比剂不良反应史或其它药物过敏史。

Exclusion criteria：

1. Patients with congenital heart disease, primary and other secondary cardiomyopathy, severe arrhythmia, frequent atrial fibrillation, and serious image quality artifacts;
2. Contraindications of MRI: there are ferromagnetic metal foreign bodies in the body (such as cardiac pacemaker, nerve stimulator, artificial metal heart valve, aneurysm clip, intraocular metal foreign body, inner ear implant, metal prosthesis, metal prosthesis, metal joint, etc.); see the informed consent of enhanced MRI for details;
3. The patients with claustrophobia had recently developed allergic diseases such as measles and allergic dermatitis;
4. Contraindications of gadolinium contrast agent: estimated glomerular filtration rate (EGFR) <30 ml/min/1.73 m2; previous adverse reactions or other drug allergy history of gadolinium contrast agent injection.

Clinical outcome, meet the guideline of novel coronavirus pneumonia diagnosis and treatment plan (trial version fifth) was confirmed by 2019-nCoV.

指标试验（即本研究的待评估诊断试验，无论为方法、生物标志物或设备，均请列出名称）：

磁共振首过灌注成像参数(Slope, TIP, PT)和延迟强化比例(%)；通过T1mapping\ T2 mapping获得量化T1\T2值?ECV及伪彩色图像?同时分析心肌的水肿及脂肪替代程度。通过MRS分析心肌肌酸代谢情况、CEST分析能量代谢及水脂分离分析脂肪替代情况。
