研究目的：

在孕早期筛选妊娠期糖尿病高危孕妇并给予预防性营养干预，最终降低孕24-28周妊娠期糖尿病发病率。  

Objectives of Study：

In the early pregnancy, screening pregnant women with high risk of gestational diabetes and giving preventive nutrition intervention, and ultimately reducing the incidence of gestational diabetes in 24-28 gestational week.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

(1) 孕妇需在国际和平妇幼保健院或新华医院或奉贤区中心医院分娩。
(2) 被GDM预测模型筛选为GDM高危人群。GDM风险概率 = 1/[1+exp(-b)]; b = (-14.2334 + (0.0681*年龄) + (0.5005* 孕早期甘油三酯, mmol/L) + (2.8165* 孕早期空腹血糖, mmol/L) + (1.1062*一级亲属糖尿病家族史) + (1.6925* HbA1c, %) +（0.4349*多胎妊娠) +（2.6181* 既往GDM史).若孕早期孕妇相关指标代入上述公式后所得风险概率≥0.13则被认为是GDM高危人群。
(3) 入组孕周在孕13+6以前。
(4) 孕妇具有阅读、理解、签署知情同意书的能力。

Inclusion criteria

(1) Pregnant women who will do antenatal care and delivered at the International Peace Maternal and Child Health Hospital or Xinhua Hospital or Shanghai Fengxian District Central Hospital;
(2) The GDM predictive model predicts this pregnant women in the high-risk group;Risk probability of GDM = 1/[1+exp(-b)]; b = (-14.2334 + (0.0681*age) + (0.5005* blood triglyceride level in early pregnancy, mmol/L) + (2.8165* fasting blood glucose level in early pregnancy, mmol/L) + (1.1062*family history of diabetes in first-degree relatives) + (1.6925* HbA1c, %) +（0.4349*multiple pregnancy) +（2.6181* personal history of GDM).The pregnant women who are selected to be at high-risk of GDM by the predictive model (calculated risk probability >=0.13) can be recruited in our study.
(3) Pregnant women before 13+6 weeks of pregnancy;
(4) Ability to read, understand and sign informed consent.

排除标准：

（1）高血压，HIV阳性、可测得的乙肝或丙肝病毒载量、乙肝核心抗原或e抗原阳性、乙肝核心抗原阳性、已知的心脏病、地中海贫血、囊性纤维化、系统性红斑狼疮等自身免疫系统疾病、大病化疗史，需要药物治疗的甲状腺疾病。
(2) 孕前糖尿病 (包括孕妇在怀孕前以诊断的糖尿病; 早孕期空腹血糖≥7.0mmol/L; 或 糖化血红蛋白≥6.5%; 典型的高血糖症状或高血糖危象症状伴任意血糖≥11.1mmol/L).
(3)目前有服用影响血糖的药物，如二甲双胍、糖皮质激素等。
(4) 已知的严重胎儿畸形。
(5) 药物滥用。

Exclusion criteria：

(1) Hypertension, HIV-positive, measurable hepatitis B or C viral load, positive for hepatitis B core or e antigen, positive for hepatitis C core antigen. Women who are known to suffer from heart disease and who have limited or no function, beta-thalassemia, cystic fibrosis,Systemic lupus erythematosus and other autoimmune diseases, History of chemotherapy,thyroid diseases requiring medication.
(2) Pregestational diabetes mellitus (including patients diagnosed with diabetes before pregnancy; fasting blood glucose >=7.0mmol/L in early pregnancy; or glycated hemoglobin >=6.5%; or typical hyperglycemia symptoms or hyperglycemia crisis with optional blood glucose >=11.1mmol/L);
(3) Currently, drugs that affect blood sugar, such as metformin and glucocorticoids, are used;
(4) Known major fetal malformations;
(5) Drug abuse
