研究目的：

1．观察术前吗啡预处理对慢性心衰患者体外循环后心肌缺血再灌注损伤及远期预后的影响。 2．探讨Cav-3/MG53及其下游蛋白激酶是否参与介导吗啡预处理心肌保护作用。  

Objectives of Study：

1. To observe the effect of preoperative morphine preconditioning on myocardial ischemia-reperfusion injury and long-term prognosis in patients with chronic heart failure after cardiopulmonary bypass. 2. To investigate whether Cav-3/MG53 and its downstream protein kinases are involved in mediating morphine preconditioning induced cardioprotection.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1) 拟在体外循环下行瓣膜置换术的换择期手术患者；
2) 年龄>18周岁，性别不限；
3) 体重指数（BMI）18.5~30 kg/m2；
4) 心衰发展到C或D阶段的患者
5) 美国麻醉医师协会（ASA）分级为II~ IV级；
6) 取得患者及家属的知情同意

Inclusion criteria

1) Patients undergoing elective surgery for valve replacement during cardiopulmonary bypass;
2) Age > 18 years old male and female;
3) Body mass index (BMI) 18.5~30 kg/m2;
4) Patients with chronic heart failure progressing to stage C or D;
5) ASA is graded II to IV;
6) Get informed consent from patients and/or their families

排除标准：

1) 既往有阿片类药物成瘾史；
2) 合并有不明原因的心肌病患者；
3) 糖尿病及肥胖患者；
4) 严重精神疾病不能配合；
5) 严重呼吸系统疾病及肝肾功能衰竭的患者。

Exclusion criteria：

1) The history of opioids addiction;
2) Patients with cardiomyopathy with unexplained causes;
3) Diabetes and obese patients;
4) Serious mental illness cannot be matched;
5) Patients with severe respiratory diseases and liver and kidney failure.
