研究目的：

针对原发肾癌经过标准化疗或靶向药物治疗，病灶控制不理想且发生脊柱转移的患者，利用基因测序分析患者基因组携带的肿瘤特异性突变并筛选合适的肿瘤新抗原，在体外利用新抗原负载患者自体DC细胞制备新抗原DC疫苗，回输患者体内以此放大肿瘤特异性新抗原信号，从而诱导患者免疫系统充分的激活，特异性地对抗患者负荷的肿瘤，以达到改善病人生存的目的。  

Objectives of Study：

For patients with spinal metastases from kidney cancer who have been treated with standard chemotherapy or targeted drug treatment with unsatisfactory focus control, gene sequencing is used to analyze the tumor-specific mutations carried in the patient's genome and screen for appropriate tumor neoantigens. Then based on the neoantigens, the patients autologous DC cells are prepared to produce neo-antigen DC vaccines. The vacines are injected back into the patients to amplify the tumor-specific neo-antigen signals, thereby inducing the patients immune system to fully and specifically activate to decrease tumor for the purpose of improvement patients' survival.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1）年龄18-70岁；
2）肺癌进展发生脊柱转移，患者有以下症状：疼痛明显或脊柱不稳或脊髓压迫；
3）肺癌经过标准化疗或靶向药物治疗，病灶控制不理想；
4）无严重的认知障碍，无感觉性失语，能够理解基本指令；
5）无心脑血管等严重基础疾病；
6）本人自愿参加本项研究，并签署知情同意书者；
7）患者预期生存期大于6月。

Inclusion criteria

1) The age was 18-70 years old;
2) Lung cancer spinal metastasis with obvious pain or spinal instability or spinal cord compression;
3 After standard chemotherapy or targeted drug therapy, the control of primary tumor is not ideal;
4) No severe cognitive impairment, no sensory aphasia, can understand the basic instructions;
5) No serious basic diseases such as cardiovascular and cerebrovascular diseases;
6) Voluntarily participated in the study and signed the informed consent;
7) The expected survival time was more than 6 months.

排除标准：

1）伴其他严重心、脑、肾、自身免疫性等疾病；
2）不配合本治疗者；
3）白蛋白水平低于30 g/L，血红蛋白水平低于70 g/L，血小板低于70000 cells/μL，白细胞低于2000 cells/μL；
4）受试者体重低于45 kg，体重指数（BMI）不在18-26 kg/m2范围。

Exclusion criteria：

1) With other serious heart, brain, kidney, autoimmune disease and other diseases;
2) Not eligible for treatment;
3)Albumin level was lower than 30 g / L, hemoglobin level was lower than 70 g / L, platelet was lower than 70 000 cells / μ L, white blood cell was lower than 2000 cells / μ L;
4)The body weight was less than 45 kg and the body mass index (BMI) was not in the range of 18-26 kg / m2.
