研究目的：

通过实施以目标为导向的护理干预措施，以期改善脑卒中患者的自我感受负担和自我管理行为，为临床提供系统、有效、可操作性的延续性护理干预方案。  

Objectives of Study：

Through the implementation of goal-oriented nursing intervention measures, with a view to improving stroke patients' self-perceived burden and self-management behavior, to provide a systematic, effective and operational continuing nursing intervention program for the clinic.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.符合1995年全国第四届脑血管病会议的诊断标准，经颅脑CT或核磁共振检查确诊为脑卒中的患者；
2.意识清楚，能以文字或语言沟通，表达清晰且能配合完成问卷；
3.病人自愿参与本研究，并同意接受出院后的随访；
4.年龄≥18岁。

Inclusion criteria

1. Patients who met the diagnostic criteria of the fourth National Conference on Cerebrovascular Diseases in 1995 and were confirmed to have cerebral apoplexy by CT or MRI;
2. Clear consciousness, able to communicate with words or language, express clearly and complete the questionnaire;
3. The patients volunteered to participate in this study and agreed to be followed up after discharge;
4. Age ≥18 years.

排除标准：

1.有严重伴随疾病，如：严重心律失常、重要脏器功能衰竭、恶性肿瘤、心肌梗死等；
2.近期遭遇其他重大应激事件；
3.意识不清或痴呆、严重认知功能障碍及精神疾病患者；
4.出院无法随访或不愿配合者。

Exclusion criteria：

1. Severe accompanying diseases, such as severe arrhythmia, failure of vital organs, malignant tumor, myocardial infarction, etc.;
2. Recently encountered other major stress events;
3. Patients with unconsciousness or dementia, severe cognitive impairment or mental illness;
4. Unable to follow up or unwilling to cooperate after discharge.

由一名研究者将96例脑卒中患者进行编号，采用随机数字表，按照入院的先后顺序，给每位患者分配一个随机数字，并将随机数字进行排序，前48位为干预组，后48位为对照组

96 stroke patients were numbered by a researcher, and each patient was assigned a random number according to the order of admission by a random number table, and the random numbers were sorted. The first 48 were the intervention group and the last 48 were the control group.
