研究目的：

分析与普通健康教育相比，健康信念模式指导下的健康教育对缺血性脑卒中患者出院后血压、空腹血糖、血脂的控制水平和用药依从性的影响，探讨可以有效提高患者遵医行为，改善预后减少疾病复发的健康教育方式，为提高患者二级预防依从性提供理论依据和方法指导。  

Objectives of Study：

To analyzed the influence of health education under the guidance of health belief model on the control level of blood pressure, fasting blood glucose, blood lipid and medication compliance of ischemic stroke patients after discharged compared with general health education. To explore the way of health education that can effectively increased patients' compliance with medical treatment, improved prognosis and reduce disease recurrence, provide theoretical basis and methodological guidance for improving patients' compliance with secondary prevention.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄≥18岁；
（2）首次发病的缺血性卒中患者，符合第四次全国脑血管病会议修订的诊断标准，并经头颅CT或MRI证实；
（3）小学及以上文化水平，具备基本的读写能力和微信软件使用能力；
（4）预计住院时间≥7天，接受充足的健康教育；
（5）预计出院时会使用降压、降糖、降脂和抗血小板聚集药物中2种及以上药物的患者；
（6）患者或家属签署知情同意书。

Inclusion criteria

(1) Aged >= 18 years;
(2) The patients with ischemic stroke for the first time met the diagnostic criteria revised by the fourth national cerebrovascular disease conference, and were confirmed by head CT or MRI;
(3) Primary school or above education level, basic reading and writing ability and wechat software use ability;
(4) The expected length of stay was more than 7 days and adequate health education was provided;
(5) Patients who are expected to use two or more of the antihypertensive, hypoglycemic, lipid-lowering and antiplatelet aggregation drugs at discharge;
(6) Patients or their families signed informed consent.

排除标准：

（1）接受溶栓或血管内介入治疗的患者；
（2）意识不清，伴有原发性精神疾病的患者；
（3）伴有其他严重脏器功能障碍或内科疾病。

Exclusion criteria：

(1) Patients receiving thrombolytic therapy or intravascular intervention;
(2) Patients with unclear consciousness and primary mental illness;
(3) Accompanied by other serious organ dysfunction or medical diseases.
