研究目的：

通过大样本的临床试验，观察星状神经节阻滞对单肺通气下腔镜食道癌根治术患者术中脑氧饱和度和术后注意网络的影响。  

Objectives of Study：

Objective To observe the effect of stellate ganglion block on cerebral oxygen saturation and postoperative attention network in patients undergoing endoscopic esophagectomy with one lung ventilation.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.择期胸科胸腹腔下（双腔管）下单肺通气手术患者，ASAI-III级；
2.年龄40-75岁；BMI 20～25kg／m2;
3.无明显心肺疾病；
4.沟通交流正常，具有一定的学习能力；
5.术前简易智能状态量表 ( MMSE) ＞27 分;
6.术前焦虑自评量表(SAS)<50分；
7.自愿受试并签署知情同意书。

Inclusion criteria

1. Patients with single lung ventilation under thoraco peritoneal cavity (double lumen tube) in thoracic department, asai-iii grade;



2. Age 40-75 years old, BMI 20-25 kg / m2;



3. No obvious cardiopulmonary disease;



4. The communication is normal and has certain learning ability;



5. Preoperative Mini Mental State Scale (MMSE) > 27 points;



6. Preoperative self rating Anxiety Scale (SAS) < 50 points;



7. Volunteer and sign informed consent.

排除标准：

1.排除患有认知功能疾病的患者，如精神障碍、老年痴呆症、中风史、帕金森氏症；
2.严重的呼吸循环系统疾病：如严重的冠心病、慢阻肺等；
3.内分泌系统疾病：如垂体瘤、严重糖尿病、皮质醇增多征、嗜铬细胞瘤等；
4.视觉或感觉障碍; 语言障碍或严重的听力障碍;
5.出血性疾病史或凝血功能障碍；
6.除外合并感染性疾病，术前炎性指标异常的患者
7.预期患者或者家属不会合作或者患者退出研究；
8.重要数据缺失。

Exclusion criteria：

1. Patients with cognitive function disorders, such as mental disorders, Alzheimer's disease, stroke history and Parkinson's disease, were excluded;



2. Serious respiratory and circulatory diseases, such as severe coronary heart disease, chronic obstructive pulmonary disease, etc;



3. Endocrine system diseases, such as pituitary adenoma, severe diabetes, increased cortisol sign, pheochromocytoma, etc;



4. Visual or sensory impairment; language impairment or severe hearing impairment;



History of coagulation disorders or bleeding 5;



6. Patients with infectious diseases and abnormal preoperative inflammatory indexes were excluded



7. It is expected that patients or their families will not cooperate or patients will withdraw from the study;

由不参加实验的一名麻醉医生应用前瞻、双盲、观察研究设计，连续纳入2021年1月至2022年12月我院220例择期行腔镜食道癌根治术的患者，随机数字表法分为G组（全身麻醉复合形状神经节组，110例），C组（全身麻醉组，110例），随机化结果被隐藏在不透明的信封中.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

1.数据的采集：本试验采用专人对入组患者资料进行收集并根据专用表格填写研究记录，用于记录受试者第一手临床试验数据资料。 2.数据的报告：CRF为统计源文件，由研究者填写。完成的CRF，第一联交统计分析单位，进行数据录入工作。第一联移交后，CRF的内容不再作修改 3.数据的监查：监查员的人数与访视频度必须满足临床试验的质控要求。 4.数据的录入及核查 （1）建立数据库：由数据管理与统计分析单位负责。 （2）核查数据：数值范围和逻辑检查，如有疑问通过监查员向研究者发出询问，研究者应尽快解答并返回，数据管理员根据研究者的回答进行数据修改，确认与录入。 5.数据可溯源性的规定：应保存质量控制性文件，如数据一致性检查，数值范围和逻辑检查的原始记录，盲态核查时的原始记录、研究者与监查员之间交流的疑问记录等。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

1. Data collection: in this study, a special person was assigned to collect the data of enrolled patients and fill in the research record according to the special form, which was used to record the first-hand clinical trial data of the subjects. 2. Data report: CRF is the statistical source file, which is filled in by researchers. Completed CRF, the first joint statistical analysis unit, data entry work. After the first copy is handed over, the content of CRF will not be modified 3. Data monitoring: the number of inspectors and the video frequency of interview must meet the quality control requirements of clinical trials. 4. Data input and verification (1) Establishment of database: it is the responsibility of data management and statistical analysis unit. (2) Check the data: check the value range and logic. If there is any question, the investigator should answer it and return it as soon as possible. The data administrator will modify, confirm and enter the data according to the investigator's answer. 5. Regulations on data traceability: quality control documents should be kept, such as original records of data consistency check, numerical range and logic check, original records of blind verification, and query records of communication between researchers and inspectors.
