研究目的：

老年性骨质疏松(SOP)是一种常见的疾病，肠道菌群失调与老年性骨质疏松的发生发展具有一定关联，但调控SOP的具体肠道菌株以及相关机制报道甚少。本课题组发现SOP患者粪便中普雷沃氏菌(P. Copri)呈优势生长,而SAMP6(快速衰老小鼠P6品系)小鼠在进行老年性骨质疏松患者粪菌移植后发现其骨质疏松程度加重，同时血清骨代谢标志物较移植前显著差异。基于此发现拟:1.研究P.Copri对SAMP6小鼠骨质疏松症和全身低度炎症的调控作用。2.研究P. Copri改变肠道内代谢产物对诱导破骨细胞的形成分化具有调控作用，应用粪菌移植技术明确P.Copri是否通过诱导破骨细胞加重SOP。3.通过体内体外上调SCFA(短链脂肪酸)的表达，阐明SCFA的减少是由于P.Copri的优势生长破坏肠道屏障功能，HAL(羟基柠檬酸内酯)易于进入体内，加速SOP的发生发展。本课题拟为SOP的防治提供新思路。  

Objectives of Study：

Senile osteoporosis (SOP) is a common disease. The imbalance of intestinal flora is related to the occurrence and development of osteoporosis in the elderly.However, there are few reports on specific intestinal strains and related mechanisms regulating SOP. Our team find that Prevotella Copri (P.Copri) in the stool of patients with SOP shows dominant growth, while in SAMP6 mice the degree of osteoporosis increases after fecal bacteria transplantation with the stool from SOP patients, and the serum bone metabolism markers are significantly different from those before transplantation. Based on this finding, the project plans to: 1.Study the regulatory effect of P.Copri on osteoporosis and systemic low-grade inflammation in SAMP6 mice. 2. Study the regulatory effect of P. Copri on the formation and differentiation of osteoclasts by changing the metabolites in the intestine, and to determine whether P.Copri aggravates SOP by inducing osteoclasts. 3. By up regulating the expression of SCFA in vivo and in vitro, it is clarified that the decrease of SCFA is due to the damage of intestinal barrier function by the dominant growth of P.Copri. HAL is easy to enter the body, which accelerates the development of SOP . This project aims to provides new ideas for the treatment of SOP

纳入标准：

年龄>65 岁； 包括 65 岁
性别： 男性
符合 WHO 骨质疏松诊断标准进入研究组。 （ DXA） ； 否则入选对照组。
本地居民至少在本地生活 10 年以上 （ 上海市浦东新区）
接受骨科相关手术术后 4 个月以上的随访患者
签署知情同意书。
能够用 Bristol Stool Scale 进行粪便分级

Inclusion criteria

1. Patients over 65 years old include patients over 65 years old;
2. Gender: male;
3. Patients who met the WHO diagnostic criteria for osteoporosis were enrolled in the study group. (DXA); otherwise, they were included in the control group.
4. Patients with local residents living in Shanghai for at least 10 years (Pudong New Area, Shanghai)
5. Patients who were followed up for more than 4 months after orthopedic surgery
6. Patients who signed informed consent.
7. Be able to use Bristol stool scale to grade feces.

排除标准：

年龄>85 岁； 包含 85 岁
5 年内出现过胃肠道大手术的， 例如： 胆囊切除术， 阑尾切除术， 小肠切除术。
炎症性肠病（ IBD） ， 包括 Crohn 病， 溃疡性结肠炎等。
持久性， 传染性胃肠炎， 胃炎； 病因不明的持续性， 慢性腹泻， HP 幽门螺旋杆菌感染（ 未治疗） 。
慢性便秘或痔疮。
历史性的胃或者肠道溃疡或胃出血， 腹痛， 消化不良， 胃肠道的里急后重，排便困难， 息肉、 包块以及癌症

Exclusion criteria：

1. Patients over 85 years old include patients over 85 years old;
2. Patients who have undergone major gastrointestinal surgery within 5 years, such as cholecystectomy, appendectomy and small bowel resection.
3. Patients with inflammatory bowel disease (IBD), including Crohn's disease, ulcerative colitis, etc.
4. Persistent, infectious gastroenteritis, gastritis, persistent unknown etiology, chronic diarrhea, HP Helicobacter pylori infection (untreated).
5. Patients with chronic constipation or hemorrhoids.
6. Patients with history of gastric or intestinal ulcer or gastric bleeding, abdominal pain, dyspepsia, gastrointestinal distress, dysdefecation, polyps, masses and cancer.
