研究目的：

1.分析血糖波动大的脆性糖尿病患者肠道菌群组成和其代谢产物SCFAs的特异性。 2.观察补充膳食纤维后，肠道菌群和其代谢产物SCFAs的变化，及其对脆性糖尿病患者的血糖波动的影响。 3.分析肠道菌群和其代谢产物SCFAs改善脆性糖尿病患者的血糖波动的作用机制。  

Objectives of Study：

1. Analysis of the composition of intestinal flora and the specificity of metabolite SCFAs in brittle diabetes patients with high blood glucose fluctuation. 2. After dietary fiber supplementation, the changes of intestinal flora and its metabolites SCFAs and their effects on blood glucose fluctuation in patients with brittle diabetes were observed. 3. Analysis of intestinal flora and its metabolites SCFAs improve the mechanism of blood glucose fluctuation in patients with brittle diabetes.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

① 脆性糖尿病（BDM）组患者符合刘新民主编的实用内分泌学（第2版）中脆性糖尿病的诊断标准；T2DM组的患者符合中国 2 型糖尿病防治指南（2013 年版）中2型糖尿病的诊断标准。
② 年龄：18-65岁。
③ BMI<24kg/m2。
④ 所有研究对象行七点末梢血糖监测（三餐前后和睡前），计算血糖标准差（SDBG）。要求BDM组SDBG≥5.4mmol/L；T2DM组SDBG≤3.0mmol/L。

Inclusion criteria

1. The patients in the group of brittle diabetes (BDM) met the diagnostic criteria of brittle diabetes in practical endocrinology (2nd edition) edited by Liu Xinmin, and the patients in the T2DM group met the diagnostic criteria of type 2 diabetes in the Chinese guidelines for the Prevention and treatment of Type 2 Diabetes (2013 Edition);
2. Aged 18 to 65 years;
3. BMI<24kg/m2;
4. All subjects underwent seven-point peripheral blood glucose monitoring (before and after three meals and before bedtime) and calculated blood glucose standard deviation (SDBG). Request BDM group SDBG>=group 5.4 mmol/L; T2DM group SDBG<=3.0 mmol/L.

排除标准：

① 肠道器质性疾病，有腹部手术史；
② 处于怀孕期或哺乳期；
③ 近2周内使用过膨胀剂、止泻药、解痉药、益生菌及抗菌药物；
④ 近2周内使用二甲双胍、阿卡波糖和DPP-4抑制剂。

Exclusion criteria：

① Intestinal organic diseases, abdominal surgery history;
② During pregnancy or lactation;
③ In the dilatants, anti-diarrhea drugs, antispasmodic drugs, probiotics and antimicrobial agents in the 2 weeks;
④ metformin, acarbose and DPP-4 inhibitors were used for nearly 2 weeks.
