Objectives of Study：

Application effect evaluation of patient guidelines based on LEARNS model in secondary prevention of ischemic stroke

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①年龄≥18岁；
②签署知情同意书；
③首次发作的急性缺血性卒中；
④轻中度卒中，且预计出院时mRS≤2。

Inclusion criteria

1. Aged >=18 years old;
2. Sign the informed consent;
3. Acute ischemic stroke in the first episode;
4. Mild to moderate stroke and mRS<=2 when expected to be discharged from hospital.

排除标准：

①无法正常沟通交流的患者；
②合并有其他心、脑、肾等严重并发症的患者；
③无法使用智能手机的患者。

Exclusion criteria：

1. Patients who cannot communicate with each other normally;
2. Patients with other heart, brain, kidney and other serious complications;
3. Patients who cannot use smart phones.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

研究者根据受试者的原始观察记录，将数据及时、完整、正确、清晰地载入病例报告表，录入采用相应的数据库系统双人双机录入，之后对数据库进行两遍比对。电子数据文件分类保存，并有多个备份保存于不同磁盘或记录介质上，妥善保存，防止损坏。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

According to the original observation records of the subjects, the researcher loaded the data into the case report form in a timely, complete, correct and clear manner. The corresponding database system was used to input the data into the form, and the database was then compared twice.Electronic data files are stored separately and with multiple backups on different disks or recording media for proper storage and protection against damage.
