研究目的：

1、头颈部肿瘤放疗患者放疗中、后会出现颈部曲度变小或者变直，摆位重复性不佳，使得剂量分布不理想、疗效下降、副反应增加，能否通过颈部肌群的训练提高颈部摆位重复性评价 2、颈部肌群训练干预，在头颈部肿瘤放疗患者中，降低摆位误差、维持颈部曲度的效果  

Objectives of Study：

1. During and after radiotherapy for head and neck tumor, patients will have reduced cervical curvature or straightening, poor positioning repeatability, resulting in unsatisfactory dose distribution, decreased efficacy and increased side effects. Whether the neck positioning repeatability evaluation can be improved through the training of neck muscle groups 2. The effect of neck muscle group training intervention on reducing the positioning error and maintaining the curvature of the neck in patients with head and neck tumor radiotherapy

药物成份或治疗方案详述：

根据患者来科前是否做过该肿瘤手术作为分层因素，将患者进行分层随机分组。 试验组：（颈椎操+推拿）+常规治疗；放疗开始前一周开始训练，颈椎操+推拿每日1次 颈椎操：分别做低头、抬头、左转、右转、前伸、后缩；顺、逆时针环绕的运动，每次坚持5分钟，每天3次 推拿：1、肌肉放松：用按揉法沿颈部双侧横突、棘突旁，放松斜方肌上部，胸锁乳突肌等颈部肌群，力道以患者感酸胀耐受为宜，约6min 2、穴位按压：疏通筋络，点按风池穴、哑门学、颈夹脊穴、落枕穴、肩井穴，约10min 3、旋转－提升活动：患者低头，手和肘部提起颈部，左右旋转并合力上提，可闻及弹响声，再次按揉放松局部肌肉，约4min 对照组：常规治疗，无特殊颈部肌肉训练干预 

Description for medicine or protocol of treatment in detail：

According to whether the patients had had the tumor surgery before coming to the department as a stratification factor, the patients were stratified and randomly grouped. Experimental group :(cervical vertebra exercise + massage) + routine treatment; Start training one week before radiotherapy, cervical vertebra exercise + massage once a day Cervical spine exercises: lower the head, raise the head, turn left, turn right, stretch forward and shrink back respectively; Do clockwise and counterclockwise circles for 5 minutes, 3 times a day Massage: 1. Muscle relaxation: The upper part of trapezius muscle and sternocleidomastoid muscle along the transverse process and spinous process on both sides of the neck were relaxed according to the massage method. The patient's tolerance to acid distension was recommended for about 6min 2. Acupressure: dredge the muscles and collaterals, and press fengchi, Mengxue, Zhujiaji, stiff neck, and shoulder well points for about 10min 3. Rotation-lift activity: the patient lowers his/her head, raises his/her neck with his/her hands and elbows, rotates from side to side and lifts the neck with joint force. The sound of spring can be heard, and then kneads and relaxes the local muscles for about 4min Control group: routine treatment, no special neck muscle training intervention 

纳入标准：

1、头颈部肿瘤放疗患者
2、年龄18至65岁
3、体力状况（KPS）评分≥70分
4、完善相关检查排除放疗禁忌
5、患者需被告知本研究的全部内容，并签署知情同意书

Inclusion criteria

1.Patients with head and neck tumor radiotherapy;
2. Patients aged 18 to 65 years;
3. Physical condition (KPS) score ≥70 points;
4.Complete relevant examination to exclude radiotherapy contraindications
5. Patients should be informed of the all contents of this study and sign the informed consent

排除标准：

1、既往接受过头颈部放射治疗者
2、既往接受过颈椎手术颈部肌肉活动受限者
3、其他颈部疾病影响肌肉活动者
4、沟通障碍、不能配合者，无法有效重复颈部肌群训练者

Exclusion criteria：

1. Patients who have received head and neck radiation therapy before;
2. Those who have received cervical spine surgery in the past and have limited cervical muscle activity;
3. Those whose muscle activity is affected by other neck diseases ;
4.Those who have difficulty in communication, cannot cooperate, and cannot effectively repeat the neck muscle group
