主要研究目的：观察研究术后心率变异性与老年骨关节手术术后谵妄的相关性 次要研究目的：观察研究术前心率变异性与老年骨关节手术术后谵妄的相关性  

Objectives of Study：

Main research purpose: To observe the correlation between postoperative heart rate variability and postoperative delirium in elderly patients undergoing orthopedic surgery. Secondary research purpose: To investigate the correlation between preoperative heart rate variability and postoperative delirium in elderly patients undergoing orthopedic surgery.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

手术类型：下肢骨科手术（包括：人工全髋关节置换、人工膝关节置换、股骨头置换、PFNA、髋关节置换翻修术、膝关节置换翻修术、股骨干骨折内固定术及胫骨平台骨折内固定术），
行蛛网膜下腔阻滞麻醉，
年龄≥60，
本人及家属签署研究知情同意书

Inclusion criteria

1. Operation type: lower limb orthopedic surgery (including total hip replacement, total knee replacement, femoral head replacement, PFNA, revision of hip replacement, revision of knee replacement, internal fixation of femoral shaft fracture and internal fixation of tibial plateau fracture);
2. Patients undergoing subarachnoid block anesthesia;
3. Patients over 60 years old;
4. The patients and their family members signed the informed consent for the study.

排除标准：

符合以下任何一项予以排除。
（i）ASA≥IV级；
（ii）最近3个月内急性冠状动脉疾病，明显的瓣膜疾病的临床证据；
（iii）心律不齐（房扑或房颤，异位搏动>正常窦性心率的5％），
（iv）内分泌疾病史（甲状腺或肾上腺疾病），
（v）中枢神经系统肿瘤病史，精神病史，癫痫病史，术前MMES评分<10或术前筛查谵妄，
（vi）严重听力障碍 且无法沟通，（vii）长期使用镇静剂或其他精神药物，滥用药物或酗酒，
（viii）术后ICU转移以进行进一步治疗

Exclusion criteria：

Any of the following will be excluded.
1. Subjects with ASA ≥ IV grade;
2. The patients with acute coronary artery disease in recent 3 months have obvious clinical evidence of valve disease;
3. Patients with arrhythmia (atrial flutter or atrial fibrillation, ectopic beat > 5% of normal sinus heart rate),
4. Patients with a history of endocrine diseases (thyroid or adrenal diseases),
5. Patients with history of central nervous system tumor, psychiatric history, epilepsy history, preoperative MMES score < 10 or preoperative screening delirium,
6. Patients with severe hearing impairment and inability to communicate,
7. Those patients who use sedatives or other psychotropic substances for a long time, abuse drugs or drink too much,
8. Patients transferred to ICU for further treatment.
