研究目的：

我们拟建立基于便携电生理技术的“家庭健康促进模式”对家庭慢性疾病患者进行管理。拟联合多家项目单位开展多中心研究，具体从以下几个方面开展： 1）以医院-医联体为依托，建立家庭健康促进中心； 2）利用便携电生理技术为手段，招募家庭慢性疾病患者及其家庭，建立方便且能有效管理患者的家庭健康促进模式； 3）在实施过程中不断培养专业人员包括电生理技术人员、临床医师、社区卫生服务人员及健康促进人员； 4）各种方式进行健康宣教，提高群众健康意识和防治慢性疾病的参与率； 5）实施项目督导和质控管理：建立家庭健康促进项目工作评估机制。  

Objectives of Study：

We plan to establish a "family health promotion model" based on portable electrophysiology technology for the management of family patients with chronic diseases.It is planned to cooperate with several project units to carry out multi-center research, specifically from the following aspects: 1) To establish a family health promotion center based on the hospital-medical alliance; 2) Use portable electrophysiological technology as a means to recruit family patients with chronic diseases and their families, and establish a convenient and effective family health promotion model for patients; 3) Continuously train professionals including electrophysiological technicians, clinicians, community health service personnel and health promotion personnel during the implementation process; 4) To carry out health education in various ways to raise public health awareness and participation rate in the prevention and treatment of chronic diseases; 5) Implement project supervision and quality control management: establish a work evaluation mechanism for family health promotion projects.

纳入标准：

1.符合家庭慢性疾病的临床诊断，包括产后尿失禁，子宫脱垂，慢性盆腔疼痛，偏瘫、中风后肌力异常；
2.宣教后，在知情同意的基础上，依从性好。

Inclusion criteria

1. Consistent with the clinical diagnosis of family chronic diseases, including postpartum urinary incontinence, uterine prolapse, chronic pelvic pain, hemiplegia, and muscle strength abnormality after stroke;
2. Good compliance on the basis of informed consent after mission and education.

排除标准：

1.不符合家庭慢性疾病的临床诊断，包括不满足产后尿失禁，子宫脱垂，慢性盆腔疼痛，偏瘫、中风后肌力异常等疾病的诊断标准；
2.在知情同意的基础上，依从性不足。

Exclusion criteria：

1. Failure to meet the clinical diagnosis criteria for family chronic diseases, including postpartum urinary incontinence, uterine prolapse, chronic pelvic pain, hemiplegia, poststroke muscle strength abnormalities;
2. Insufficient compliance on the basis of informed consent.
