研究目的：

通过本随机对照研究可初步阐明湿化高流量鼻导管氧疗（humidified high flow nasal cannula，HHFNC）用于急诊抢救室中低氧血症和呼吸困难患者对呼吸频率、患者耐受性评分、舒适度评分、血气分析结果、转归及预后、气管插管率、死亡率及并发症发生率等的影响，明确湿化高流量鼻导管通气在急诊抢救室中低氧血症和呼吸困难患者中应用的优势，为急诊抢救室中低氧血症和呼吸困难患者的早期氧疗方式的选择提供临床数据支持。  

Objectives of Study：

Through this randomized controlled study preliminary clarify wetting high flow oxygen therapy nasal catheter (humidified high flow nasal cannula, HHFNC) used in emergency intensive care patients with low oxygen concentration and difficulty breathing in patients with respiratory frequency and tolerance grade, comfort, blood gas analysis results, outcomes and prognosis, endotracheal intubation rate, mortality and incidence of complications and the effect of clear of wetting high flow ventilation in emergency intensive care nasal catheter in patients with low oxygen concentration and difficulty breathing the advantage of application,To provide clinical data support for the selection of early oxygen therapy for patients with hypoxemia and dyspnea in emergency department.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

(1)年龄≥18岁，出现急性呼吸困难伴低氧血症和呼吸困难（不吸氧情况下，呼吸频率≥24次/分，脉搏血氧饱和度≤94%）；
(2)无明显的口咽活动性出血、无明显的排痰障碍；
(3)意识清醒；
(4)受试者自愿参加研究并己签署知情同意书；

Inclusion criteria

(1) age ≥18 years old, acute dyspnea with hypoxemia and dyspnea (without oxygen, respiratory frequency ≥24 times/min, pulse oximetry ≤94%);
(2) no obvious oropharyngeal active bleeding, no obvious obstruction of sputum discharge;
(3) consciousness;
(4) the subjects voluntarily participate in the study and have signed the informed consent;

排除标准：

(1) 存在有创通气指征，达到必须尽快行气管插管及有创呼吸机辅助呼吸的标准；
(2) 积极液体复苏及血管活性药物使用后血流动力学不稳定患者（收缩压<90 mmHg、平均动脉压<50 mmHg）；
（3）意识严重减退(Glasgow昏迷评分13分以下)；
(4)拒绝配合积极治疗或拒绝参加本研究；
(5)不能清除呼吸道分泌物的；
(6)恶性肿瘤病史且明显影响此次预后；
(7)严重室性心律失常或心肌缺血;
(8) 口咽、头面部、气道及胸腹腔存在尚未纠正的活动性出血。

Exclusion criteria：

(1) the presence of invasive ventilation indications, must be intubated as soon as possible and invasive ventilator assisted breathing standards;
(2) patients with hemodynamic instability after active fluid resuscitation and vasoactive drugs (systolic blood pressure <90 mmHg, mean arterial pressure <50 mmHg)
（3）The Glasgow coma scale is below 13.
(4) refuse to cooperate with active treatment or refuse to participate in this study;
(5) unable to remove respiratory secretions;
(6) Malignant tumor history and significantly affect the prognosis;
(7) severe ventricular arrhythmias or myocardial ischemia;
(8) there is active bleeding in oropharynx, head, face, airway and chest and abdomen, which has not yet been corrected.
