研究目的：

输尿管软镜碎石取石术已成为治疗肾输尿管结石（<2cm）的一线首选方案。但是因为输尿管狭窄狭小导致软镜鞘置入输尿管失败率约为8％～20％。采用输尿管镜直视下应用直视球囊对输尿管狭窄部位进行扩张置入软镜鞘后行软镜碎石的方法可行，且患者不需二次手术，也无需面临机械扩张致输尿管误损伤和增加射线暴露的风险。应用直视球囊是否能提高一期软镜的成功率是本项研究的目的。  

Objectives of Study：

Ureteroscopic lithotripsy has become the first-line choice for the treatment of renal ureteral calculi (< 2cm). However, due to narrow ureter, the failure rate of soft endoscope sheath implantation in ureter is about 8% ~ 20%. It is feasible to dilate ureteral stricture with balloon under ureteroscope and insert soft endoscope sheath and then perform soft endoscope lithotripsy, and the patient does not need secondary operation, and does not need to face the risk of ureteral accidental injury and increased radiation exposure caused by mechanical dilatation. The purpose of this study is whether the application of direct vision balloon can improve the success rate of primary soft endoscopy.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.肾盂、各肾盏、输尿管上段结石，长径<2.0 cm，ESWL处理困难、失败或残留（定位困难、结石较硬或停留时间长的嵌顿性结石等）。
2.极度肥胖、严重脊柱畸形，不适合经皮肾镜取石术者。
3.伴盏颈狭窄的有症状的肾盏憩室内结石患者。

Inclusion criteria

1. In patients with renal pelvis, calyces and upper ureteral calculi, the length and diameter are less than 2.0 cm, and ESWL is difficult to handle, fail or remain (difficult to locate, hard calculi or incarcerated calculi with long residence time, etc.).
2. The patients with extreme obesity and severe spinal deformity are not suitable for percutaneous nephrolithotomy.
3. Patients with symptomatic calyceal diverticulum stones with calyceal neck stenosis.

排除标准：

特殊人群（18-70周岁以外患者和孕妇）；
直径大于2CM的肾结石、肾内感染者和感染型结石；
一期对侧需腔内治疗（经皮肾镜碎石取石术、输尿管硬镜碎石取石术、RIRS）；
术前影像学确定输尿管有3CM以上狭窄和输尿管闭锁患者；
肿瘤压迫及侵犯导致的恶性输尿管狭窄；
肾移植患者；
无法停止的抗凝药物应用；
出血倾向；
严重心肺功能不全；
严重髋关节畸形的患者。

Exclusion criteria：

1. Special population (patients beyond 18-70 years old and pregnant women);
2. Patients with renal calculi larger than 2cm in diameter, patients with renal infection and infectious calculi;
3. One stage contralateral patients (percutaneous nephrolithotomy, ureteroscopic lithotripsy, RIRS);
4. Patients with ureteral stricture above 3cm and ureteral atresia confirmed by preoperative imaging;
5. Patients with malignant ureteral stricture caused by tumor compression and invasion;
6. Renal transplant patients;
7. Patients who cannot stop the use of anticoagulants;
8. Patients with bleeding tendency;
9. Patients with severe cardiopulmonary insufficiency;
10. Patients with severe hip deformity.
