研究目的：

（1）制定“患者为核心、社区医师为执行主体、三级中医院专科医师为主导”的东部中心城市5 种心脑血管疾病三级防控规范； （2）开展中医慢病管理模式的应用研究，评价管理成效，优化服务规范，形成5种心脑血管疾病“家庭-社区-医院”三级防控示范基地。  

Objectives of Study：

Formulate the three-level prevention and control specifications for 5 kinds of cardiovascular and cerebrovascular diseases in the eastern central city with "patients at the core, community physicians as the executive body, and three-level TCM hospital specialists as the leading"; Apply research, evaluate management effectiveness, optimize service standards, and form a three-level demonstration base for prevention and control of "family-community-hospital" five types of cardiovascular and cerebrovascular diseases.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄≥18岁；
（2）符合心绞痛、心肌梗死、心力衰竭、脑梗死、脑出血疾病诊断标准；
（3）患者自愿签署书面知情同意书，能配合完成研究工作。

Inclusion criteria

(1) Aged >= 18 years;
(2) Meet the diagnostic criteria for angina, myocardial infarction, heart failure, cerebral infarction, and cerebral hemorrhage;
(3) The patient voluntarily signs a written informed consent form and can cooperate to complete the research work.

排除标准：

（1）正在参与其他慢病管理项目者；
（2）严重精神系统疾病，或其他重大疾病引起的精神障碍；
（3）严重失语、失认、失用者；
（4）根据研究者判断患者不能完成本研究或不能遵守本研究要求（由于管理方面原因或者其他特殊原因）

Exclusion criteria：

(1) Those who are participating in other chronic disease management projects;
(2) Severe psychiatric diseases, or mental disorders caused by other major diseases;
(3) Severe aphasia, ignorance, or apraxia;
(4) According to the investigator's judgment, the patient cannot complete the study or cannot comply with the requirements of the study (due to management reasons or other special reasons).
