研究目的：

本项目旨在通过随机对照的临床观察，探索特利加压素是否能有效减少肝切除术中的出血量，促进患者术后加速康复 。  

Objectives of Study：

The purpose of this project is to explore whether terlipressin can effectively reduce the amount of bleeding during hepatectomy and promote the recovery of patients after hepatectomy through randomized controlled clinical observation.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1)年龄18~65岁，性别不限；
2)ASA Ⅰ~Ⅱ级，心功能1级；
3)术前临床诊断为原发性肝癌，术后病理证实为肝细胞肝癌、胆管细胞癌或混合性细胞癌；
4)行择期部分肝切除术（残肝体积＞ 40%）；
5)肝功能为Child-Pugh分级：A级，或B级经术前治疗改善至A级；
6)术前肾功能正常。

Inclusion criteria

1) The age ranged from 18 to 65 years old;
2) ASA Ⅰ - Ⅱ, cardiac function grade Ⅰ;
3) Preoperative clinical diagnosis was primary liver cancer, postoperative pathology confirmed as hepatocellular carcinoma, cholangiocarcinoma or mixed cell carcinoma;
4) Partial hepatectomy (residual liver volume > 40%) was performed;
5) Liver function was child Pugh grade: Grade A or grade B was improved to grade A after preoperative treatment;
6) Renal function was normal before the operation.

排除标准：

1)3级以上高血压病或1~2级高血压病未控制（服用降压药后日常血压仍>140/90mmHg)；
2)有心脑血管疾病（冠心病，脑梗塞病史等）；
3)肝功能Child分级B级以上，肾功能异常（BUN>9mmol/L, Cr>178μmol/L)
4)BMI>30kg/m2;
5)已知对特利加压素或硝酸甘油过敏患者;
6)有活动性肝炎表现；
7)肝癌合并大血管侵犯,及肝癌复发再次手术治疗患者；
8)活动性消化道出血、 肝功能障碍、凝血功能严重减退并且无法纠正，或其他特利加压素禁忌症患者。

Exclusion criteria：

1) Hypertension above grade 3 or 1-2 was not controlled (daily blood pressure was still > 140 / 90mmHg after taking antihypertensive drugs);
2) Cardiovascular and cerebrovascular diseases (coronary heart disease, cerebral infarction history, etc.);
3) Liver function child grade B or above, abnormal renal function (BUN > 9mmol / L, Cr > 178 μ mol / L)
4)BMI>30kg/m2;
5) Patients known to be allergic to terlipressin or nitroglycerin;
6) There was active hepatitis;
7) Patients with HCC complicated with vascular invasion and recurrence of HCC were treated by reoperation;
8) Patients with active gastrointestinal bleeding, liver dysfunction, severe blood coagulation failure that cannot be corrected, or other terlipressin contraindications.

选取 2020 年7月 1 日至 2021 年 2 月 28 日病理诊断为肝癌的行肝脏切除手术的病人共 96 例，1:1 为比例随机分为试验组和对照组组.
