主要目的：观察超声引导下前锯肌平面阻滞（SAPB）联合患者自控静脉镇痛（PCIA）对于开腹肝部分切除手术的镇痛效果及安全性，探讨其对患者早期康复的影响。  

Objectives of Study：

Main purpose: To observe the analgesic effect and safety of ultrasound-guided serratus anterior plane block (SAPB) combined with patient-controlled intravenous analgesia (PCIA) for open partial hepatectomy, and to explore its effect on early recovery of patients.

药物成份或治疗方案详述：

纳入标准：

选取2020年10月-2022年10月于我院择期行开腹肝部分切除手术患者110例，性别不限，年龄18~80岁，BMI 18 ~ 30 kg/m2，ASA Ⅰ—Ⅲ级，肝功能child-pugh A或B级，无精神病史、听力障碍、语言障碍。

Inclusion criteria

A total of 110 patients who underwent elective open partial hepatectomy in our hospital from October 2020 to October 2022 were selected, regardless of gender, aged 18 to 80, BMI 18 to 30 kg/m2, ASA Ⅰ-Ⅲ grade, liver Function child-pugh grade A or B, no history of mental illness, hearing impairment, or language impairment.

排除标准：

具有麻药过敏史，拟穿刺部位感染，凝血功能障碍，外周神经系统疾患，患者或家属拒绝行神经阻滞，慢性疼痛史，长期服用镇痛、镇静药物或酗酒、吸毒。

Exclusion criteria：

Has a history of anesthetic allergy, infection at the site to be punctured, coagulation dysfunction, peripheral nervous system disorders, the patient or family members refuse to undergo nerve block, a history of chronic pain, long-term use of analgesic, sedative drugs or alcohol or drug abuse.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

1. 数据安全监察计划 ①项目风险等级评估：低风险。理由：试验风险稍大于最小风险；国内外多份报告指出，前锯肌平面阻滞可以用于术后镇痛。 ②拟定监察频率：神经阻滞后半小时内每10分钟监察一次，注意观察患者生命体征和反映情况，注意预防神经阻滞相关并发症；术后2、6、12、24、48 h进行监察，并记录相关数据。 ③CRA（数据监查员）：由我科王迪副研究员担任，独立于本研究之外，王迪参与过多次临床研究，并对前锯肌平面阻滞有过相关的临川研究，由他监查和报告试验的进行情况和核实数据。 2. 不良事件（AE）的观察 所有不良事件均详细记录，恰当处理并追踪直到妥善解决或病情稳定，按照规定及时向伦理委员会、主管部门和药品监督管理部门报告严重不良事件（SAE）等。 主要研究者定期对所有不良事件进行系统性回顾，必要时召开研究者会议评估研究的风险与受益；大于最小风险的研究将安排独立的数据监查员对研究数据进行监查，以做出研究是否继续进行的建议。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

1. Data Security Monitoring Plan ① Project risk level assessment: low risk. Reasons: The trial risk is slightly greater than the minimum risk; multiple reports at home and abroad pointed out that serratus anterior plane block can be used for postoperative analgesia. ②Proposed monitoring frequency: monitor the nerve block every 10 minutes within half an hour, pay attention to the patient’s vital signs and reflections, and prevent complications related to nerve block; monitor 2, 6, 12, 24, and 48 hours after surgery , And record relevant data. ③CRA (Data Monitor): Associate researcher Wang Di of our department, independent of this research, Wang Di has participated in many clinical studies, and has related Linchuan research on serratus anterior plane block. He monitors and reports the progress of the test and verifies the data. 2. Observation of Adverse Events (AE) All adverse events are recorded in detail, properly handled and tracked until they are properly resolved or the condition is stable, and report serious adverse events (SAE) to the ethics committee, competent authorities and drug regulatory authorities in a timely manner as required. The main investigator regularly conducts a systematic review of all adverse events, and when necessary, convenes an investigator meeting to evaluate the risks and benefits of the research; for studies greater than the minimum risk, independent data monitors will be arranged to monitor the research data to conduct research Suggestion whether to proceed.
