研究目的：

随着我国人口老龄化，髋部骨折患者逐年增加。指南建议患者应在骨折后48h内手术，但患者从入院到手术前的时间却是疼痛管理的空窗期，缺少持续有效的镇痛措施，术前检查过床、转运等体位变动又加剧骨折的疼痛，导致疼痛相关不良反应的发生，尤其是增加围术期心脑血管事件和认知功能障碍发生的风险，严重影响患者预后。 本课题利用区域阻滞镇痛技术，在患者急诊入院后立即行超声引导下髂筋膜间隙阻滞，并留置导管持续镇痛，通过将镇痛时间窗前移，从而改善患者围术期疼痛、生理、情绪、精神状态、认知功能和满意度等，提高术后恢复质量。  

Objectives of Study：

With the aging of population in China, the number of hip fracture patients is increasing year by year.Guidelines recommend that patients should be within 48 h after fracture operation, but the patients from hospital to preoperative time is window period of pain management, lack of effective analgesic measures continuously, preoperative examination bed, a transit position changes and fractures in pain, and lead to pain related adverse reactions, and especially increase perioperative cardiovascular events and the risk of cognitive dysfunction, seriously affect the prognosis of patients. This topic using regional block analgesia in patients with emergency immediately after admission line of ultrasound guided iliac fascia space block, and the indwelling catheter continuous analgesia, by putting the analgesia time window to move, to improve patients' perioperative pain, physical, emotional, mental status and cognitive function and satisfaction, improve the quality of postoperative recovery.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄≥65岁，
（2）ASA分级Ⅱ、Ⅲ级，
（3）无视力、听力、语言功能障碍；

Inclusion criteria

1. Subjects aged over 65 years old;
2. Subjects of ASA grade II and III;
3. Subjects without visual impairment, hearing impairment or language impairment.

排除标准：

（1）患者有神经系统或精神系统疾病，
（2）有慢性疼痛病史，
（3）有麻醉药物过敏史,
（4）凝血功能障碍,
（5）穿刺部位感染。

Exclusion criteria：

1. The patient has neurological or psychiatric disorders;
2. Patients with a history of chronic pain;
3. Patients with a history of allergy to narcotic drugs;
4. Patients with coagulation dysfunction;
5. Patients with puncture site infection.
