研究目的：

1. 评估二甲双胍、右美沙芬能否有效缓解肿瘤进展，是否影响肿瘤细胞的干性以及肿瘤微环境； 2. 探索二甲双胍、右美沙芬影响肿瘤发生发展的机制。  

Objectives of Study：

1. Evaluate whether metformin and dextromethorphan can effectively alleviate tumor progression through affecting the stemness of tumor cells and tumor immune microenvironment or not. 2. To explore the mechanism of metformin and dextromethorphan affecting tumor growth and development.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 自愿同意参与研究并签署知情同意书；
2. 年龄在18-75岁之间（包含），性别不限；
3. 食管癌：具有切除可能性食管鳞癌患者；
4. 血压﹤160/100 mmHg；
5. 血甘油三酯﹤8 mmol/L

Inclusion criteria

1. Volunteer to participate in the study and sign the informed consent form;
2. Age from 18 to 75 years old, no gender limitation.
3. Esophageal cancer: The patients with esophageal squamous cell carcinoma who have the possibility of resection.
4. Blood pressure < 90/50mmHg
5. Blood triglycerides ﹤ 8 mmol/L

排除标准：

1. 肝、肾功能不全的患者：基线天冬氨酸转氨酶（AST）或者丙氨酸转氨基酶（ALT）≧3.0倍 ULN（正常值上限），和/或血清肌酐（SCr）>1.5 mg/dl。如患者入组前出现实验室检查异常值，应与研究者讨论决定其是否入组；
2. 怀孕或哺乳期妇女；
3. 合并有其他恶性肿瘤并在接受积极治疗；
4. 不稳定型心绞痛，未控制的缺血性心肌病或有症状的充血性心力衰竭（例如：纽约心脏协会功能分级的三或四级）；
5. 慢性肝病或肝硬化；
6. 体循环血压较低的患者(血压<90／50mmHg)或未控制的高血压患者(血压>170／110mmHg)；
7. 甲肝、乙肝、艾滋病、结核等传染性疾病或结缔组织病处于活动期者；
8. 严重感染尤其是肺部感染者；
9. 合并有严重口腔疾病（严重牙周炎及口腔肿瘤等）的患者；
10. 不具备知情同意的能力；
11. 入选前1年内参加过或正在参加其他临床试验的患者；
12. 已知对二甲双胍、右美沙芬过敏或不能耐受。

Exclusion criteria：

1. Patients with hepatic and renal insufficiency: AST and ALT exceeding 3 times of the normal upper limit and CCR≤ 50ml/min.
2. Pregnant or lactating woman.
3. Combined with other malignant tumors and receiving active treatment.
4. Unstable angina, uncontrolled ischemic cardiomyopathy or symptomatic congestive heart failure (eg., grade III or IV of the New York Heart Association functional grading).
5. Have a history of lactic acidosis.
6. Patients with low blood pressure (blood pressure < 90/50mmHg) or uncontrolled hypertension (blood pressure > 170/110mmHg).
7. Infectious diseases such as hepatitis A, hepatitis B, AIDS, tuberculosis or desmosis are in active stage.
8. Severe infections, especially those in the lungs.
9. Patients concurrent with severe oral disease (severe periodontitis and oral tumors, etc.).
10. Didnt have the ability of being informed the consent.
11. Patients who have participated in other clinical trials within one year prior or are participating in other clinical trials.
12. Known to be allergic or intolerant to metformin or dextromethorphan.
