研究目的：

胸科手术PND的发生率明显高于其他非心脏手术，PND的发生不仅会增加住院时间和医疗费用，还会导致术后恢复延迟、并发症增多，降低患者生活质量。本研究拟评价椎旁神经阻滞对老年胸科手术患者术后认知功能的影响。  

Objectives of Study：

The incidence of thoracic surgery PND is significantly higher than that of other non-cardiac surgery. The occurrence of PND will not only increase hospitalization time and medical expenses, but also lead to delayed postoperative recovery, increased complications and reduced the quality of life of patients. This study intends to evaluate the effect of paravertebral nerve block on postoperative cognitive function in elderly patients undergoing thoracic surgery.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

年龄≧65岁；ASA分级Ⅱ—Ⅲ；择期行肺癌、食管癌手术。

Inclusion criteria

1. Aged >= 65 years;
2. ASA grade II-III;
3. Elective surgery for lung and esophageal cancer.

排除标准：

罹患神经精神疾病不能配合者，术前存在认知功能损害（MMSE评分<27分）；
有重大心、肺、肝、肾及内分泌或血液疾病者；
有慢性胸痛患者，正在进行慢性疼痛治疗者（吗啡≧30mg/d或其他阿片类药物当量）；
有药物过敏史（阿片类药物、局麻药、NSASIDs）。

Exclusion criteria：

1. Preoperative cognitive impairment (MMSE score <27);
2. Significant heart, lung, liver, kidney and endocrine or blood disorders;
3. Chronic chest pain patients undergoing chronic pain treatment (morphine >= 30 mg/d or other opioid equivalents);
4. History of drug allergy (opioid, local anesthetic, NSASIDs).
