研究目的:

痰标本采集是临床重要的诊疗手段。在ICU每位重症肺炎患者在气管插管后、病情恶化和拔管前都需要留取痰标本,做为诊断疾病的依据。目前临床使用的开放式留取痰标本法,会使患者产生剧烈呛咳,痰液飞沫喷溅污染周围物表,同时产生带大量致病微生物的气溶胶,致使医护人员处于病源暴露的风险中 。并且气道压的大幅度波动对危重病人机体氧合和肺顺应性也会有不利影响。为了减少留取痰标本时,致病微生物随着痰液经气管插管喷溅或产生气溶胶导致院感发生,保护医务人员和病人免受院内感染,我们设计了“密闭式留取痰标本装置”,联合目前使用的密闭式吸痰清理气道的方法,有望达到此目的。  

Objectives of Study:

Sputum specimen collection is an important means for recognize the pathogen of those patients with respiratory infection. For patients in the ICU who require respiratory supported by ventilator, sputum specimens for pathogen monitoring are frequently required, especially when the patient's condition is deterioration. When endotracheal suctioning during mechanical ventilation, each patient would produce intense choking cough lead to sputum droplets splashes, with a large number of pathogenic microorganism aerosol floating in the air. The patients and medical staffs in the same room have to suffer from the pathogenic exposure risk. Moreover, the traditional procedure of collecting sputum specimens inevitably leads to large fluctuations in airway pressure of critically ill patients, result in Oxygen saturation and pulmonary compliance deterioration. In order to reduce the incidence of hospital infection caused by the pathogenic microorganisms spraying with sputum or producing aerosol through endotracheal intubation during sputum collection, and to protect medical staff and patients from nosocomial infection, we designed a closed sputum collection device, which is expected to achieve this purpose by combining with the currently used closed endotracheal sputum suction method to clear the airway.

纳入标准:

①已签署知情同意书,同意参加本项目研究;
②使用有创呼吸机的细菌性肺炎患者(OI<250mmHg);
③气管插管时间>3天 。

Inclusion criteria

Informed consent has been signed to participate in the study of bacterial pneumonia patients, who need ventilator with tracheal intubation, duration of ventilation support > 3 days .(OI<250mmHg)
