研究目的：

1.1 前瞻性评价应用开放获取网站Vancomycin-Calculator.com计算的万古霉素初始剂量在我国老年患者中应用的适用性及安全性。 1.2 与既往经验性应用的万古霉素初始剂量进行对比，比较应用Vancomycin-Calculator前后老年患者万古霉素初始剂量达标率及安全性，为老年患者更合理有效应用万古霉素提供参考。  

Objectives of Study：

1.1 Prospective evaluation of the applicability and safety of the initial dose of vancomycin calculated by the open access website Vancomycin-Calculator.com in elderly patients in my country. 1.2 Compared with the previous empirically applied vancomycin initial dose, compare the vancomycin initial dose compliance rate and safety of elderly patients before and after the application of Vancomycin-Calculator, and provide a reference for the more reasonable and effective use of vancomycin in elderly patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄≥65岁；
（2）因确诊或疑诊MRSA等耐药G+菌感染应用万古霉素治疗；
（3） 至少有一次初始剂量治疗后的万古霉素血药谷浓度检测结果。

Inclusion criteria

(1) Age ≥ 65 years old;
(2) Vancomycin should be used for treatment of confirmed or suspected MRSA or other drug-resistant G+ bacteria infection;
(3) At least one result of trough blood concentration of vancomycin after initial dose treatment.

排除标准：

（1）万古霉素口服或持续静点治疗；
（2）静脉应用万古霉素＜3次，不能进行血清谷浓度检测；
（3）应用万古霉素前已存在严重慢性肾功能不全（CCr＜20ml/min）或出现肾损伤（诊断标准：48h内血清肌酐值升高≥26.5μmol/L（0.3mg/dL）或≥基础值的1.5倍，或每小时尿量＜0.5mL/kg，持续时间超过6h ）；
（4）治疗前或治疗过程中接受血液透析、腹膜透析或血液滤过等肾替代治疗；
（5）缺乏身高、体重等指标，不能进行万古霉素初始剂量计算；
（6）缺乏临床感染及实验室评价指标，不能进行安全性评价

Exclusion criteria：

(1) Oral vancomycin or continuous intravenous treatment;
(2) Intravenous use of vancomycin <3 times, the serum trough concentration cannot be detected;
(3) Severe chronic renal insufficiency (CCr＜20ml/min) or renal injury occurred before vancomycin application (diagnostic criteria: serum creatinine value increased within 48h ≥26.5μmol/L (0.3mg/dL) or ≥ 1.5 times the basic value, or urine output per hour <0.5mL/kg, duration more than 6h);
(4) Receive renal replacement therapy such as hemodialysis, peritoneal dialysis or hemofiltration before or during treatment;
(5) The initial dose of vancomycin cannot be calculated due to lack of indicators such as height and weight;
(6) Lack of clinical infection and laboratory evaluation indicators, and safety evaluation cannot be carried out
