慢性阻塞性肺疾病(COPD)是一种慢性呼吸系统疾病,其特点是持续的进行性气流受限,是慢性发病率和死亡率的主要原因,据《柳叶刀》杂志报道,中国40岁以上人群的发病率从2008年的8.2%上升到2015年的13.7%[1],与1990年相比,2015年全球慢性阻塞性肺病死亡率增加了11.6%[2]。其发展过程经常发生急性加重,加重被定义为呼吸系统症状的急性恶化,导致额外的治疗[3-4]。COPD急性加重(AECOPD)占所有COPD相关费用的70%以上[5],是患者临床病情恶化的主要原因。研究表明,COPD导致肌力、基本生活活动能力和肺功能显著下降,进一步损害患者健康相关的生活质量(HRQoL),增加患者对病情加重、住院和死亡的易感性[6-7]。因此,COPD患者的治疗目标是尽量减少这些事件的负面影响,防止其复发。肺康复治疗(PR)是一种对慢性阻塞性肺病患者的有效干预措施[8]。 肺康复(Pulmonary rehabilitation, PR)是COPD患者非药物治疗的重要组成部分,包括但不限于运动训练、教育和自我行为管理。它在改善症状、运动能力、生活质量和预后方面有显著效果[9-10]。 阻力训练是一种减少运动过程中代谢负荷和呼吸困难的方式,使训练比耐力训练更容易忍受[11-12]。对于呼吸困难和/或运动肌无力的患者来说,阻力训练具有趣味性。在肺康复过程中的阻力训练被认为是必不可少的,而且可能比传统的耐力持续训练更重要[13]。阻力训练对促进骨骼肌肉力量,运动能力和COPD生活质量有显著效果[14]。此外,在COPD患者中观察到I型纤维减少、I型和II型纤维萎缩、毛细血管减少和代谢酶水平改变,并可通过耐力训练显著改善[15-19]。因此,阻力训练被认为是COPD患者康复干预的重要组成部分[20]。弹力带可用于慢性阻塞性肺病患者作为重量机械设备的替代,因为材料的成本更低,需要更少的物理空间[21]。它的优点是在家庭环境可以促进执行和易于获得[22-23]。这一点特别吸引人,因为在全球范围内,这种常见疾病的治疗要求很低,因此弹力带可能成为增加肺部康复治疗的一种选择[24-27]。此外,弹力带能够从收缩开始到运动结束线性地增加肌肉的张力,而且对关节的损害更小[28]。另外,与传统的重量机械设备相比,弹性带可能使得更多的肌肉激活和使用更快的神经运动系统的收缩运动单元[29]。一些研究已经证明了弹力带作为COPD的一种训练方式的积极作用,但仍处于早期研究阶段。本研究旨在探讨弹力带训练对稳定期慢性阻塞性肺疾病患者的康复效果。 |

Objectives of Study: |
Chronic obstructive pulmonary disease (COPD) is a chronic respiratory disease, characterized by a continuous progressive airflow limited, is a major cause of chronic morbidity and mortality, according to the lancet reported that the incidence of China more than 40 people, up from 8.2% in 2008 to 13.7% in 2015 [1], compared with 1990, 2015 chronic obstructive pulmonary disease (COPD) a 11.6% increase in mortality in the world [2].Acute exacerbations occur frequently in the course of development, which is defined as an acute exacerbation of respiratory symptoms leading to additional treatment [3-4].Acute exacerbation of COPD (AECOPD) accounts for more than 70% of all COPD related costs [5], and is the main cause of patients' clinical deterioration.Studies have shown that COPD leads to significant decreases in muscle strength, basic living activities, and lung function, further impairs patients' health-related quality of life (HRQoL), and increases patients' susceptibility to exacerbations, hospitalizations, and deaths [6-7].Therefore, the therapeutic goal for PATIENTS with COPD is to minimize the negative effects of these events and prevent their recurrence.Pulmonary rehabilitation therapy (PR) is an effective intervention measure for patients with chronic obstructive pulmonary disease [8]. Pulmonary Rehabilitation (PR) is an important component of non-drug therapy for COPD patients, including but not limited to exercise training, education, and self-behavior management.It has significant effects in improving symptoms, motor ability, quality of life and prognosis [9-10]. Resistance training is a way to reduce metabolic load and dyspnea during exercise, making training more bearable than endurance training [11-12].Resistance training is interesting for patients with dyspnea and/or motor weakness.Resistance training is considered essential in pulmonary rehabilitation and may be more important than traditional endurance continuous training [13].Resistance training has significant effects on promoting skeletal muscle strength, motor ability, and COPD and quality of life [14].In addition, reduction of type I fibers, atrophy of type I and II fibers, decreased capillaries, and changes in metabolic enzyme levels were observed in COPD patients, which could be significantly improved through endurance training [15-19].Therefore, resistance training is considered to be an important part of rehabilitation intervention for COPD patients [20].Elastic bands can be used in patients with CHRONIC obstructive pulmonary disease as an alternative to weight-mechanical equipment because materials are cheaper and require less physical space [21].It has the advantage of facilitating implementation and easy access in a home environment [22-23].This is particularly attractive because, globally, treatment requirements for this common disease are low, and elastic bands may therefore be an option to increase pulmonary rehabilitation [24-27].In addition, elastic bands can linearly increase muscle tension from the beginning of contraction to the end of movement, with less damage to joints [28].In addition, elastic bands may enable more muscle activation and use of contractile motor units of the neuromotor system faster than conventional weight-mechanical devices [29].Some studies have demonstrated the positive effects of elastic bands as a form of training for COPD, but are still in the early stages of research.The purpose of this study was to investigate the rehabilitation effect of elastic band training on patients with stable chronic obstructive pulmonary disease. |
