研究目的:

研究旨在通过CTA技术与Mimics数字化软件相结合,达到以下研究目的: 1)通过精细化三维重建的方式重建旋股外侧动脉、降支及其分支,重建主要穿支血管相对于周围骨骼、肌肉、皮肤三者的稳定三维可视化模型; 2)通过对三维可视化模型分析、精确标记转移到患者的体表进行定位,在患者体表进行设计重现,从而完成股前外侧皮瓣的个性化设计,标记皮瓣切取的范围,模拟皮瓣的切取,为手术的顺利开展提供精准的导航,避免对外科医生对经验的盲目依赖; 3)将手术过程透明化及简单化,缩短手术时间,降低因皮瓣血管变异带来的手术风险,提高头颈部组织缺损的修复成功率和满意度。  

Objectives of Study:

The research aims to achieve the following research objectives through the combination of CTA technology and Mimics digital software: 1) To reconstruct the lateral circumflex artery, descending branch and its branches by means of refined THREE-DIMENSIONAL reconstruction, and to reconstruct the stable THREE-DIMENSIONAL visual model of the main perforating vessels relative to the surrounding bone, muscle and skin; 2) based on 3 d visualization model analysis, accurate marker is transferred to the patient's body surface to locate, in patients with body surface to carry on the design, personalized design, so as to complete the femoral anterolateral flap tag flap for the scope of the simulation of the flap cut, provide operation smoothly with precise navigation, avoid blindly rely on for the surgeon to experience; 3) Make the surgical procedure transparent and simple, shorten the operation time, reduce the surgical risk caused by vascular variation of the flap, and improve the success rate and satisfaction of the repair of head and neck tissue defects.

纳入标准:

① 年龄18-65周岁,性别不限;
② 头颈部有骨、软组织暴露,或肿瘤切术术后经植皮不能修复的创面;
③ 可以完全理解手术风险并能自主签署知情同意书及伦理同意书。

Inclusion criteria

1. Patients aged 18-65 years, regardless of gender;
2. Patients with bone and soft tissue exposure in the head and neck, or wounds that cannot be repaired by skin grafting after tumor resection;
3. Patients who can fully understand the risk of surgery and can sign informed consent and ethical consent independently.

排除标准:

① 儿童、小于18岁的青少年、孕妇及身体机能明显减退的高龄患者;
② 碘过敏试验阳性及肾功能有异常的患者;
③ 有严重气质性疾病不能耐受手术者;
④ 有精神性疾病,不能自主签署手术知情同意书者;

Exclusion criteria:

1. Children, adolescents younger than 18 years old, pregnant women and elderly patients with obvious physical decline;
2. Patients with positive iodine allergy test and abnormal renal function;
3. Patients with severe temperament diseases who can not tolerate surgery;
4. Patients with mental illness and unable to sign the informed consent of operation independently.

所有图像由2名从事股前外侧皮瓣工作4年及以上的医师以双盲法阅片,对患者病灶的位置、形态、范围等是否受到侵犯、侵润及是否转移进行评估,意见出现分歧时共同阅片统一意见;

All images are reviewed by 2 physicians who had been working on the anterolateral thigh flap for 4 years or more with a double-blind method. The position, shape and scope of the lesion are evaluated for invasion, invasion and metastasis.
